Question: my wife has itchy vulva and perineum also pain in those areas

Thanks for writing.
Candida infection is commonly responsible for majority of the infection.
Itching is most often seen in candidiasis.
a vaginal pessary of clotrimazole at bedtime like candid V for 6 days will relieve the pain.
A cream of clotrimazole and mild steroid like candid B to be applied on perineum twice a day for a week for itching.

Sorry I have done that already (injected the cream over a 3 day course - twice) no help, also lots of pain around the vagina vulva after intercourse, and my peis iches also.

Thanks for writing.
If you have already been treated for candidiasis then following causes can be the reason.
Mixed infection with other organisms. This will require a pelvic examination by a gynaecologist and infection screen and treatment with oral antibiotics like metrogyl 500 mg thrice a day for 5 days and aziththromycin 2gm single dose.
Lubricant use like lubic jelly during intercourse as lack of lubrication during intercourse can cause minor abrasion and itching.
