Question: Hello.. I have small red spots around 15 of then under my foreskin and not my glans yet.. It happened aftr sexual intercourse when I just slept off and did not clean my penis.. It itches with some discharge and are red spotty..

As per details, the most likely possibility for your symptoms is a condition of skin called Candidial balanitis.
It is fungal infection of skin involving the genital area characterised by red colored itchy rash over genitalia.
In such cases,oral fluconazole is the choice of treatment.

Thankyou for your response. i was confused wether it was bacterial or fungal infection.. i have been using neosporin ointment before and it used to fade away.. but this time i thought of not using neosporin anymore and just let it be the way it would heal naturally... the spots are only under my foreskin and it really pains when i pour cold water instead of warm water... my partner does not seem to have any of this at all its only me... sometimes i feel like excessive alcohol intake accompanied with sexual intercourse can be another reason for this.. this is the 4th time im experiencing this from the past one year. before i got the red spots i had a slight burning sensation wen i urinate but now it doesnt burn at all except for the red spots under my foreskin when i pull it back.. im sure this will fade away in time but i just want to be sure of what ointment/gel should i be using for this.. the red spots are only under my foreskin (posthis) and not my glans (round shaft). is neosporin gel ok? or do u suggest something else?

Yes as per details, it looks like fungal infection.
I usually advice my patients to apply topical cream containing clotrimazole or miconazole twice daily to be applied over the affected area. Oral fluconazole course for 2 weeks is also advised.
You may need antihistaminics like levocetrizine for symptomatic relief.
Keep the area clean and dry.
Visit your doctor so that he ca give you prescription.

Thankyou for your response ... Another thing is that my body is a heaty type.. I've got more than a 100 pimple scars only on my back due to my diet since childhood which include spices, masalas, everyday meat like checken, lamb, beef, pork, etc... So even now if I eat anything with a lot of masala wether non veg or veg, I still sweat a lot even if I eat in a cold air conditioned place... Sometimes I end up getting acne on my face forehead chest etc.. For all that I just apply pure lime juice over my back for the scars and pimple on the face ... I will change the medicine to what you recommended I.e oral flucanazole and cream containing clotrimazole.... Should I really take the antihistaminics? What is symptomatics relief? I hope it has nothing to do with herpes or std's!!!

Symptomatic relief means the irritation or itch that you have been experiencing would be relieved.
For pimples, I usually advice topical benzoyl peroxide and clindamycin gel.
If severe, oral antibiotics are also given like Doxycycline / minocycline / azithromycin.
Avoid oily food in diet.

Hello again. I was not Clear on what i had to ask you previously when we had a chat doctor. My girl and I had some pretty rough sex sessions a week ago and I had a spot that was rubbed raw just below the line on my penis and under my foreskin when pulled back on the right and extreme left underforeskin side. . It scabbed over, but the scab kept coming off. 2 days later, a small patch of blisters formed underneath the sore. Then a few days ago, more little blisters began to form in the area as well. it had a slight yellowish discharge with a foul smell on it. i kept cleaning it every hour. After 2-3 days the blisters have seemed to scab over, while the sore is slowly starting to heal. It appears that the blister like rash was slowly spreading. Is this herpes , friction burn , or another type of reaction (Candidal Balanitis or yeast infection like you said)? The blisters itch , and gets painfull only while fiddling with it or if i touch the affected area. I Always Thought yeast infection only occurs to females in the vaginas and not men.

I have a few Queries that i wish to clear for my own personal knowledge if in case it appears again in future i would try using these remedies:-

Doubt :- Is candidal balanitis the other term for a yeast infection? and is yeast and fungal infection both the same?? please correct me.

1. Is Coconut Oil good for Candidal Balanitis/ Yeast infection?

2. Is Apple Cider Vinegar good for the same?

3. Is Nebasulf powder which contains (neomycin, bacitracin and sulphateamide) good to be applied over the affected area?

4. Is "Neosporin" ointment which contains ( Neomycin, polymyxin b sulphates and bacitracin zinc ) good for the affected area. i think "neosporin" is both anti-bacterial and Anti-fungal right?? maybe the Polymyxin b sulphate is an anti-fungal ingredient in that ointment?? please correct me on this.

5. Today i washed my penis with warm salt water for around 10 minutes. It definitely did feel very nice and good. but is it fine to wash the penis with warm salt water?? After that i Patted it dry and applied a little coconut oil on it and sprinkled some nebasulf powder what i mentioned above.

6. Can you please advice me on how to differentiate between what exactly a Bacterial Infection is and What a Fungal Infection is or looks like?? So next time if i have the same type of red spots/Blisters/very light yellowish discharge of the same type should i just consider it to be a fungal/yeast infection without any doubts???

7. I am very Sure and Certain that i got this everytime (only 4 times in my life this being the 4th time from the past one year) after having unprotected sex with my gf, but if thats the case even she should be having the yeast infection. but she doesnt seem to be having any kind of such problem. I also wanted to know if after having unprotected sex, i dont wash off my penis till the next morning, can that also be a reason for this candidal balanitis due to smegma infection or something. Please correct me on this.

8. Also i wanted to tell you that i have a heaty body.. so i remember most of the times that i experienced this candidal balanitis was somewhere related to drinking a lot of alcohol in the previous days. so does alcohol have direct relation to this infection due to my heaty body, alcohol, unprotected rough sex and not washing it till next morning?

Now the description you have given goes against the diagnosis of fungal infection.
In medicine, both history and clinicak examination are very important aspect of making a diagnosis.
As per details, there are following possibilities for your symptoms :
1 ) Traumatic - at times vigorous sex leads to erosion or friction blisters over the skin of genitalia. They may subsequently resolve spontaneously. In case of secondary infection with bacteria, it leads to yellow colored pus formation and discharge.
in such cases, I prefer to advise topical antibiotic cream like mupirocin 2 percent over the affected area. If severe, oral antibiotics are also given like cloxacillin.
2) Herpea progenitalis : it is viral infection of skin involving the genitalia. It is characterised by grouped vesicles on erythematous base or multiple grouped erosions. It is a highly recurrent condition.
It can be acquired by sexual contact.
In such cases, oral antivirals like Acyclovir / Valacyclovir is started within 24-48hours of eruptions of vesicles. It may also discharge yellow pus when infected secondarily.

Both the conditions may lead to formation of scab during the course of healing.

Regarding your queries , treatment is given depending on the type and extent of rash.
Fungal infection can infect both males and females.
You may go for IgG and IgM antibodies for Herpes simplex virus 1 and 2.
It will differentiate between trauma and viral infection.
You should visit your doctor so that he can examine you and order test for you.

Hello again...

I think you have misunderstood my query or maybe i wasn't that clear about the issue doctor..

The Spots have almost dissappeared and was anyways dissappearing from the time it was on its peak i.e only 2 days without me applying anything on it.. and the 'yellowish dishcharge' that i had mentioned was only for 2 days only when the red spots were at its peak and it was not from my penis where we urinate from.. it was from those small red spots itself which stopped soon.. so it was just the puss from those spots..

And if it was actually genital herpes then wouldnt my partner also be having symptoms of those, but thats not the case at all for her doctor.

From my previous queries, what was it exactly that made you think that it could be genital herpes?

Anyways now it has almost dissappeared.. it only occurs after a rough sexual session mostly otherwise i have never got any of these rashes at all..any i have been sexually active with only one person who seems to be completely alright and has none of these problems at all.

besides , if you can write me down some common symptoms to be sure that its genital herpes, then atleast i can procede further with my medications etc.

Herpes is a condition as I already mentioned which is characterised by red colored erosions or XXXXXXX blisters. It is usually associated with itch or pain or burning sensation.
It has a self limiting course. The very first episode settles in 10-15 days and subsequent episodes settle within a week even without treatment. There is formation of blisters which usually last for 2-3 days followed by healing. The discharge may occur from these blisters as such as they are fluid filled or as a result of secondary infection. The partner may not necessarily manifest the symptoms.
I gave you two possibilities as you mentioned your symptoms of Traumatic or Herpetic origin.
Here I can only advise you about the most possible diagnosis. For confirmation of the diagnosis, clinical examination and investigations are necessary.
You should consult your doctor in person so that he can examine you.
