What Causes Itchy Rashes On Fingers Ad Palms?

default
Posted on Wed, 5 Aug 2015
Question: I have, I assume a Rash on my hands its progressing and my Dr. doesn't know what it is! it started 4-5 moths ago on the inside of one of my fingers then to the palm on same hand. Now its on all the inside on all my fingers and on palms of my hands! usually if left alone it doesn't itch but if irritated it itches so bad Id like to chop of my hands! Going back I did remember having an itch on my shin on my leg which is still there too. I have noticed it on back of my hand and now traveling proximal in my arms! This looks like nodules on my fingers and softer bigger bumps in my palms. The skin if now lookn dried up on some parts. My Dr. has tried triamcinolone ace, clotrimazolen & betamethasone, terbinafine hydrochloride (I think this was for a rash on under arms-fungus-cured!), & now permethrin cream! Im going crazy and it looks like my finger capillaries are getting dark red although my cap refill time is normal. my fingers are getting stiffer! HELP PLZ Anyone!
Not a question but a follow up info. My fingers have nodules (kinda hard) my leg and arms have tiny bumps like goose bumps, and the palms of my hands have approx. large 1-3 cm circle reddened soft padded singular bump / lefting / raised skin! I have some itching currently on sternum of chest but no signs of bumps, red spots just itch. The goose bumps on back of my hand seem to come and go (rise and fall) during the day. I have nvr had any allergies nor am I allergic to any meds or substances. I hope this helps!

Brief Answer:
Possible Dyshidrotic Eczema.

Detailed Answer:
Hi Sir,

I understand your query and concern,

I have gone through uploaded images.

You possibly seem to have Dyshidrotic eczematous eruption.

This can run a chronic and fluctuating course .

Dyshidrotic eczema treatment can be quite challenging because of the severe inflammatory process that can be involved and because of frequent recurrences.

Pharmacologic treatment in your case should begin with high-strength, topical corticosteroids and short course of systemic corticosteroids since you seem to have recalcitrant lesions which have not responded to triamcinolone and betamethasone.

Administration of Cotrimoxazole,Triamcinolone and Permethrin is not advisable in your case since the process is hardly of any infectious origin.

I advise you to go for topical application of Clobetasol cream.
Apply thin layer to affected areas twice daily and rub in gently and completely; not to exceed 50 g or 50 ml /week.

Clobetasol is for severe episodes. It is a super potent topical steroid; it suppresses mitosis and increases the synthesis of proteins that decrease inflammation and cause vasoconstriction.

Simultaneously take Tab Deslor 10 mg once a day for 15 days.

The mainstay of treatment is Prednisone. It is usually given for short course -12 days in your case ,starting with high dose and then quickly tapering the dose.

I recommend you to take the tablet after breakfast at 5-7 am daily on fixed time say at 6 am.

The dose is as follows-
(With 10 mg tablets).

Day 1 take 6 tablets
Day 2 take 6 tablets
Day 3 take 5 tablets
Day 4 take 5 tablets
Day 5 take 4 tablets
Day 6 take 4 tablets
Day 7 take 3 tablets
Day 8 take 3 tablets
Day 9 take 2 tablets
Day 10 take 2 tablets
Day 11 take 1 tablet
Day 12 take 1 tablet .
Day 13---STOP THE DRUG.

Please consult your doctor with regards to above treatment.

Avoiding foods rich in nickel and cobalt, since they can aggravate these skin lesions.
Ex -canned foods, foods cooked using nickel-plated utensils, herring, oysters, asparagus, beans, mushrooms, onions, corn, spinach, tomatoes, peas, pears, rhubarb, tea, cocoa, apricots, beer, beets, cabbage, cloves, chocolate, coffee, liver, nuts, scallops, and whole grain flour .

This regimen should be of great help.

Post your further queries if any,
Thank you.
