What Causes Itchy Rash In Groin Area?
Question: I noticed a small, flat round purplish spot on the glans of my penis about 4 weeks ago. 2 weeks ago I developed a pimply looking rash on my thighs/groins/balls. The rash was mildly itchy but not too bad... I figured it was jock itch. I went to my primary care physician, he felt it was probably jock itch but noted the red bumps were unusual. He prescribed me econozol ointment. I've XXXXXXX using it for one week now and the rash has improved some.
The spots on the penis were never addressed and i want to know what it is. I noticed purple/reddish discolored circle on left top of glans 4 weeks ago as well as purple coloring on top center of glans and about 3 small reddish circles on right side. Flat, not ichy or painful.

I have reviewed the Images.
The groin rash is definitely fungal, more specifically candidal intertrigo.
I can see a bright red rash in groin folds and there are satellite lesions around it. This is classical of candidal intertrigo.
The rash on your glans too is a candidal infection i.e candidal balanitis.
Candidal infection of glans usually presents as macerated/ moist papules which rupture and leave behind red, superficial erosive lesions with moist scaling.
There may be mild itching or no symptoms at all.
I suggest you to use the same topical antifungal cream i.e econazole nitrate over the glans as well.
An Oral antifungal e.g weekly fluconazole tablet, for 4-6 weeks would be more effective in controlling both the groin rash as well as candidal balanitis.
Oral antifungals are prescription drugs and therefore I suggest that you talk to your doctor for the needful.

This does not look like any sort of std right? I shiuld have no concern of that? My doctor never prescribed me oral antifungals, will this clear with just the cream?

You may use topical antifungals, twice daily, for a week. They should alone prove effective in resolving candidal balanitis
However, oral antifungals would clear it faster and more completely.

What has me so concerned is spots occured after rough handjob and protected sex with stranger. My doctor tested for hiv, chlymedia and gonnareah.. All negative. He aaid it looks nothing like herpes and did not warrant testing for it.

Thanks for your help, last question. I have been treating the jock itch on my thighs/groin with econozole for 12 days now and it has gotten better, still there but not as bad.
I began using the econoZole cream on the spots on my glans 4-5 days ago and have not noticed any difference in the spots. How long do you think i should be using the cream for before i see some progess on these spots?
Would you advice seeing a doctor again if the spots dont respond to the cream? Or should i assume its nothing and just move on and stop worrying?

In my patients of suspected candidal balanitis, in addition to the topical antifungal cream I would normally also suggest an Oral antifungal i.e fluconazole 150 mg single dose for candidal balanitis. Together these would be more effective.
These spots on glans are actually due to the Inflammation that accompanies candidal infection and therefore once the infection is controlled the spots would also resolve.
The spots should resolve within a week of topical and Oral antifungal treatment.
Since groin involvement is also quite extensive therefore weekly fluconazole 150 mg for 4-6 weeks is a treatment of choice, in addition to the topical antifungal cream.
Oral fluconazole is a prescription drug therefore you may talk to your doctor for a prescription regarding the same.
