What Causes Itching On Back, Around Nipples, Limbs And Scrotum?

Question: I had itching over my back and then around my nipples. then it spread to my arms and legs. I was advised Tenovate (gsk) clobetasol cream IP. this helped and most of the itching has gone.but now the itching is around my private part (around scrotum). I also feel some small bois under the skin near my scrotum. though I am using the same cream, do I need to have antibiotics? if so what kind of antibiotics?
