What Causes Irritation On The Tip Of Penis Post An Unprotected Oral Sex?

Question: I had unprotected oral sex with a stranger in July 2015. Immediately following the encounter, I felt a burning sensation at the tip of my penis, which remained the next morning. Since the encounter, I have experienced a sensitivity in the tip of my penis when it rubs against my boxer shorts. Using boxer briefs has mostly relieved this problem, but not completely. I have never experienced any painful urination, painful ejaculation, or discharge from my penis. The head of the penis seems overly sensitive to rub, but it is not necessarily painful to the touch. The slight irritation with my penis rubbing against my boxer shorts while walking is the only remaining symptom. The irritation appears to be focused at the meatus. Nothing is noticeable while sitting or not moving. I have been with one partner for six years and have experienced significant anxiety from this event.

I have had the following test, all with negative results:
HIV Early Detection @ 2 weeks
10 Test Panel @ 10 weeks
10 Test Panel @ 16 weeks
HIV Antibody @ 13 weeks
Mycoplasma Genitalium NAA @ 6 months
Antifungal Cream Treatment for 3 weeks
Urologists Office Urinalysis and subsequent "culture," both negative.

My General Physician thought he saw some slight irritation inside the urethra, which lead him to suspect M. Gen. I have taken the following medications:
Azithromycin 500mg once a day for four days
Augmentin 875-125mg twice a day for ten days
Doxycycline 100mg twice a day for 10 days
Levofloaxacin (levaquin) 500mg once a day for fourteen days.
My urologist took a quick look and said that my penis looked completely fine, which conflicted the information my GP gave me. I visited my GP again after the urologist and he confirmed that the urethra still looked slightly irritated. I plan on getting one more M.Gen test to confirm the previous results.
At this point, frustration and depression are starting to sink in as I lose hope in solving this issue. I have not had sex with my girlfriend in months out of fear of passing something. I am looking for ANY possibility about what could be going on.

- rough or dry sex
- candida infection which is common

With regards to M.genitalium, it is suspected in cases of chronic urethritis. The diagnosis is made based on the test you have already done. If resulted negative, then, no fear for this infection.

To my opinion, there might be other explanations to the irritation you are feeling:
- still thinking of candida infection as possible cause. Oral treatment should be considered together with local therapy (I was reviewing the treatment you had and no oral medication for candida infection was carried out). But before, I suggest to culture the urethral swab for bacteria and fungal infection.

- post-sex friction can be another possibility

- concentrated urine (especially if you do not drink too much water) if urine analysis and culture was negative for infection

- if not circumcised, it is also common to get urethritis; once you get circumcised, your problems will also be over.

Yes, candida infection can persist for six months due to ineffectiveness of the medications used or hostile strain of candida that is difficult to treat.

As I was reviewing your medical history, I noted that you have not used any treatment for candida (whether orally or topically). Can discuss this further with your doctor too, but first consider running swab culture to rule out candida.

Other causes (as mentioned above) should be also considered.

Ok, and I want to clarify that the sensitivity appears to be isolated to the meatus, not necessarily the outer skin of the penis.

Sorry for the late reply due to my XXXXXXX clinical consultations.

Having the sensitivity appearing to be isolated to the meatus is also indicating irritation due to dry/hard sex and possible urethral infection (bacterial/candida infection that is determined by swab culture) or due to irritation from foods/drinks that make your urine more concentrated and frequent (e.g. caffeinated drinks/alcohol, etc.).

If the outer skin of the skin is not involved, your problems are located to the urethral opening due to the causes as mentioned above.
