Question: what does ALERT !!! RBCS : mARKED ANISOCYTOSIS MODERATE POIKILOCYTOSIS, PREDOMINANTLY NORMOCYTIC NORMOCHROMIC WITH MICROCYTES, OVALOCYTES MEAN. MY HB IS JUST 7.7 AND I HAVE A MARKED DEFICIENCY OF VITAMIN D. I HEREBY ENCLOSE MY REPORTS PLEASE SUGGEST WHAT ARE THE PROBLEMS I HAVE AND WHAT SHOULD BE DONE TO RECTIFY THEM. i AM GAYATHRI XXXXXXX AGED 34, HEIGHT 152 CM, WEIGHT 80 KG, MOTHER OF A TWO YEAR OLD TODDLER. I AM ON THYRONORM 100MG. ON THE DAY OF TEST FASTED AND DID NOT TAKE THYROID PILLS.

I understand your concern..

From the reports provided, you have an iron deficiency anemia. The cause of iron deficiency should be determined. Do you have heavy bleeding? Is there any bleeding from rectum? Get a stool routine to check for occult blood in the stool.
Get a urine routine to see for hematuria. If there is heavy bleeding, get a scan done to see for fibroid uterus.

Calcium is mildly reduced. Vitamin D is markedly reduces. Take food rich in Vit D like fish, fortified yogurt/milk, exposure to sunshine. You can take vit D supplements which is available as sachets.

HbA1c is marginally increased and fasting glucose level of 140 is high. So there is onset of diabetes. Get a repeat FBS and PPBS and start on antidiabetic tablet as per the advise of a physician.

LDL is increased and HDL/LDL ratio is increased. Get a good diet control and repeat fasting cholesterol after 6 months. If still high, medication might be require. For height, you are overweight, reduce the weight to 52 kg which is ideal for your height.

Anti microsomal antibodies is high. It means there is inflammation of thyroid.. But all the rest of thyroid values are under control. Repeat AMA after 6 months to see for rising/decreasing titre..

I had heavy non stop bleeding one year ago and took three months progesterone tablets. For the past 6 months my periods r perfect and no bleeding in stool or urine so far.
U have said onset of diabetes ... Does it mean i should start medicine n continue throughout my life? Is there any control measures without meds? How can i improve my overall health without meds cos am only 34. And for thyroid inflammation will it require surgery as i am a teacher and will it affect my voice?

Thanks for writing..

Since you had no obvious bleeding, anemia might be due to heavy bleeding which occurred earlier. It would be better to deworm yourself and your family by taking a course of albendazole. It would be better to take 2 doses of iron sucrose injection and then follow up with iron tablets to bring up hemoglobin. Take iron rich food like dates, papaya, liver etc.

You have got fasting blood sugar of 140 which should be below 110 ideally. Get a repeat FBS and PPBS. Only based on that we can decide whether diet control will be sufficient or you will have to take antidiabetic medicines. Diabetic medicines once started have to be taken lifelong..

To improve overall health you need to reduce your weight to 52 kg.. Strict diet containing more of green leafy vegetables and fruits and less of fatty food and fried items.Timely food in small quantity is advisable. Starving is not advisable. Daily exercise for 30 min. Aerobics helps to reduce weight faster. On reducing weight both sugar level and cholesterol level comes down..

Right now no surgery is required for high AMA. 6 monthly checkup for AMA and follow up with a surgeon is advisable. Rarely only thyroid surgery causes vocal nerve injury. So dont worry..
