Question: hi im pregnant for 2 wks and have spotting till now its bleeding like period but less.. can pregnancy come period?

Periods do not come during pregnancy.
There are two common causes of bleeding during early pregnancy.

1. Implantation bleeding, ussually occurs just at the time of expected period and is mild and stops by itself.

2. threatened or inevitable abortion in both these conditions the baby tries to abort and hence there is bleeding. A sonography is needed to look for the condition of the baby. If the baby is OK then progesterone and bed rest are helpful in stopping the process. Sometimes the process has progressed beyond recovery [ inevitable aboriton], here a surgical evacuation procedure is needed other wise there may be heavy bleeding.

Other causes for bleeding are cervical polyps [ non cancerous growth of cervix] and erosion of cervix.

However any case of pregnancy with bleeding needs urgent investigation. The first is an examination followed by an ultrasound to look fro the well being of the baby.

So if you are having bleeding please go to see your doctor as soon as possible.

hi dr ive check with my gynae.. she cant seems to see anything beside the thick blood implementation linning.. myb too early.. yesterday my hcg is 486.. tomorrow I will do hcg again.. is tgere any such cases..? isit normal?.. my bleeding is normal lik period but less..

Hello and welcome back,

It may be implantation bleeding. This occurs when the baby attaches to the uterus and some space is left that is still not occupied by the baby. So this space bleeds till the baby obliterates it. It tends to settle down by itself but we prefer to provide support with progesterone and injection HCG, just in case it may be a threatened abortion.
If thickened endometrium is seen, wait for two more weeks for the next scan. The HCG levels will decide about the fate of pregnancy. If they are increasing then there should not be any worries. Just take bed rest, folic acid tablets, progesterone and HCG after consulting your doctor.
Please let me know the first day of your last menstrual period if possible.

These cases are very common. So please do not worry. As of now it may be an implantation bleed or an abortion. The HCG and scans will decide it. More likely it looks like an implanation bleed. So just take the treatment and let me know the HCG levels as well as you last menstrual period if possible.

I understand your concern. But unfortunately these are the two most probable diagnosis at the moment and we need to wait for the results of the tests for further confirmation.

my gynae provide me with duphaston 10mg from last fri 25th april for the pregnancy support n as well as folic acid from last 2mths.. all take once daily..

on 12th april I did test before my wisdom tooth surgery and it was negative.. on 20th april I test it was positive.. technically its 1wk 2 days.. but if count from first day of last period which is 21 march is 5wk plus.. I have the spotting from 17th april till 24th april.. 25th april starts to have red fresh blood like period.. now the blood slightly darken..

I hope n really hope this time round it can be a successful one.. coz I fail last yr prenancy.. 3mths its not growing..the hcg level does not increase double but increase 2000 each 48hr only..

Hello and welcome,

The pregnancy is always counted for the first day of the last menstrual period. So if the first day was 21 March you are 5 weeks 4 days as of today.

A gestational sac is seen by a transvaginal scan at around 5-6 weeks or at HCG levels above 1500-2000. So you can wait for 1 weeks more and repeat the scan to see if a sac is seen. Sometimes if the pregnancy is conceived late then the sac may appear late. So we prefer a scan between 6-7 weeks to include patients with delayed conception.

You are already taking duphaston and folic acid. However bleeding is unfortunately never an encouraging sign. So please take bed rest and hope for the best.

I completely understand your concern as you have suffered once before with similar issues. However as of now, you are already on all possible drugs to help the pregnancy. A rishing HCG even if it is not doubling is a positive sign. So wait till the next scan for more positive confirmation.
