What Causes Increase In Ferritin Level?

Question: Dear Sir, I'm a healthy person who sports 4-5 times a week and eat very healthy. Lots of fish and chicken and less meat. 4 years ago I have had a blood test and came out that I had a high gamma gt (80 and normal was 50 as stated) and this was due to my diet and alcohol consumption as well as light smoking. I was referred to an ultrasound and it came out that I had a light / mild fatty liver. Also my cholesterol was high. so I decided to change my life style, so I started work outs and running for 4-5 times a week. I lost 10 kg and became 75 kg from 85 (I am 171 cm tall) and all values came normal since then. My cholesterol, lever .. etc.. I check blood every year and for this year something weird came out ( at least I think weird) My ferritin (I dont know if this was ever checked by a test) is 324 while it's stating 17-230 for men.. My hb is 8.4 while it had to be minimum of 8.5 (but maybe this is because I sport too much). I am suffering now 3 weeks of a mild cough and lots of stress. the stress and cough are becoming better now. Also I have eczema and nail fungal infection on foot. ( just for information to help you giving your opinion). so if this high Ferritin was from this condition, would go up quickly and then drop down slowly? I will attach my report as all other test came out normal. Crp, lever values, thyroid, Glucose ...etc.. all came normal. Do I have to worry much about this value of ferritin? I have my fears and scares for this now. Note that there is no known family history (father, mother) history of hemochromatosis in the family. I dont eat much red meet as I eat fish almost everyday in the afternoon and most of time chicken in the evening. I also used last year daily 1 x protein shakes as it is necessary for building the muscles The shake I was using was containing protein but no cholesterol or extra iron One more update, I'm consume alcohol in the weekend.. Mainly red wine. Like 5-9 glasses of wine in one weekend Value of ferritin indicated is in μg/l If you have any question please let me know. Thanks for your time. Regards. XXXX

Brief Answer: Infection most probable reason. Detailed Answer: Hello! Thank you for the query. Ferritin belongs to the proteins which get elevated during inflammation in the body (this group of proteins is called acute-phase proteins). Its task in case of an inflammation is binding iron and inhibiting microbes iron uptake. So in case of any inflammation, this protein will suddenly rise in the blood stream. Stress can also cause inflammation-like reaction and lead to ferritin release to the blood stream. So according to described by you recent cough and stress, this seems to be the most probable reason of such result. Hemoglobin can also get decreased a little during an inflammation. Second reason of elevated ferritin is hemochromatosis indeed. This should be considered if after the and of infection, ferritin still will be elevated. Here is what you should do: - wait a week or so, and recheck ferritin level along with iron level - if it will be elevated, you should consult hepatologist and consider liver biopsy - if the cough will be still present, chest X-ray should be done Please also note that sometimes this protein can be also elevated in case of cancer. Hello, thanks for your answer. my gp called me today and told me that I dont have to be worried at all. My HB is 8.4 and that is not a bad value because some websites are mentioned 8.1 as a minimum value. since I sport a lot it can be due to that (as i read on internet). 4 years ago my ferritin was little high as well (about 260 or something and I think that was when I had an ultra sound where a very slight fatty liver was detected and fatty liver is found a lot in my country of origin). And he told me that as I am from an Asian origin, this might be due to that as higher ferritin levels are found in Asian people. He suggested (as I was worried), to do the test again in 3 months to see. he didnt expect anything. At the other hand, my ferritin is only 324 and some website are indicating a XXXXXXX of 300 and some even 400.. So doing a liver biopsy for this reason is a big operation I think . As i told you I had a cough and really huge of anxiety and stress the last month, you told me that this could be the reason. But you didnt tell me how quick this can raise and how quick this can get down after the infection or stress and how stress can effect HB and Ferritin... My liver values are perfect. Did you have a look to my report? all other values as well. You also didnt tell me if little more than normal intake of protein through shakes would raise the ferritin as well. Please check my note regarding this on my first message. You scared me more when you mentioned cancer. I dont think I have cancer or at least I don't hope !!! Thanks in advance for your answer.

Brief Answer: . Detailed Answer: In my answer I have tried to point out the important facts which you should focus on. I didnt mention protein intake as a reason of elevated ferritin as it has nothing to do with it. Increased protein intake can affect kidneys, but does not cause elevated ferritin level. Elevated ferritin 4 years ago could be due to some infection or stress as well. Explanation that you are from Asian origin what CAN (but does not have to) be the reason of elevated ferritin is not acceptable. Please note that if this would be the reason in your case, ferritin level would be constantly elevated. Your ferritin at level 324 is higher than it should be. This is the fact. Please note that every laboratory can have a different referrential ranges. It depends of the method they use and calibration of the instruments. So as your laboratory says that the level should be below 230, your level is high. I didnt say go and do the liver biopsy. I just mentioned it as a an option in case of ferritin level wont get back to normal. And like I have mentioned biopsy should be considered by a Hepatologist. Please note that many hemochromatosis cases stay undetected for many years. Mostly because of ignoring some symptoms (like constant weakness, stomach issues, gray skin) and lab results. That is why I have asked you to check iron level (yes I have checked your report where iron level is not present). Ferritin can appear suddenly (like I have mentioned before). It means that it can appear in few hours. Ferritin stays in blood stream for about 7 days. That is why I have asked you to redo this test after a week. Stress for immune system can be exactly the same as infection. So ferritin can get increased due to stress in the same way it does when infection appears. Hemoglobin level can get decreased during infection and due to stress as well. You hemoglobin is only slightly decreased which can be an effect of dehydration for example (hematocrit level should be also checked to verify this). I didnt mean to scare you with cancer possibility. But I have tried to give you all important information. You are quite young person so the cancer is hardly possible. thanks. One more question.. I also have eczema. Would that be a reason? What did you mean that if the asian would increase then it would be constantly elevated. Your explanation about how ferritin increase and decrease make sense. I thoguht ferritin was iron.. What is the test which shows iron level? And can you tell me something about that? I mean in case of ferritin is high and iron level is ok? what does that mean? Thank you In case that there was an infection when I did the test.. The CRP is low (1).. Is that not strange?

Brief Answer: . Detailed Answer: No, eczema is rather not the reason. Eczema is a kind of autoimmune/allergic inflammation, where bacteria or viruses are not involved. So it does not cause ferritin increase (ferritin is secreted to the blood stream to bind iron, such iron is not accessible for bacteria). If some person (Asian for example) has different protein level because of origin, this means that such person has it elevated all the time. We can compare it to the high. For example if Asian person is shorter than European, this person is high all the time, not from time to time. Same situation is with ferritin level. If you have it higher than other persons because you are Asian (for example), with every lab test, ferritin is higher than referential ranges. Feritin is a protein. Please note that iron which is not connected with protein, is toxic for human body. So to make it non-toxic, it must be connected with ferritin. More iron in the body, more ferritin must be produced. Situation where ferritin is high and iron is normal can appear due to inflammation/ stress, cancer (hardly possible at your age). You do not need any special test for iron level. Every laboratory can do it. CRP is not always elevated due to infection. It also increases slower than ferritin. Moreover we are talking about mild throat/lungs infection, so CRP and ferritin can act like that. Hope this will help. Regards.

Well this is what I mean.. Because my ferritin was higher in 2010 as well as now.. So if the next one is on the same level or less but still high then I have to be referred ? Or still monitored? Because I also read on Internet that a value of 1000 of ferritin is a concern or between 500 and 1000 Please note that my C-reactive protein is normal which excludes infection or not?

Brief Answer: . Detailed Answer: But you do not know how was ferritine acting between 2010 and now. It has never been rechecked. So if the next level will be still high, please have iron level checked. If iron will be elevated as well, consult hepatlogist. If iron will be normal, you should have chest X-ray to see if there is no inflammation there (as you have had some cough). If nothing wrong will be there, we can assume that you have higher ferritin than other persons because of your origin. Hope this will help. Regards.
