Posted on Sat, 9 Aug 2014
Question: Sir,
I am having spasm in neck since 2 months.
It started when I had sudden sneezing and was not able to move the neck.
Neck movement was restricted.

Consulted Doctor and tool Ultracent .I had vomiting and stopped the medication.
Took Pregalin 650( 1 dose) and had giddiness and stopped the medication.
Neck movement was there to some extent and was not severe.

Had physiotherapy exercise for once ( Pressing the head down by the therapist).
Was then not able to swallow the food , and there was tong tightness and felt like something was stuck in the neck . Their Heaviness in breathing.

Consulted Doctor after 2 days and took Hifenac on the first day and Tolperisone 150 mg ( one dose ) . I then had loose motion on the second day. Stopped the medication.
Tongue movement was observed tight on the right side. Food slowing was difficult.

Went to native place and had blood test done on 13 / 06 / 2014.
As per the report Vitamin D was < 3, Calcium was 8.3 , B12 was 238 and Hemoglobin was 11.9.

Came back to my place after 10 days and went to doctor.
Took Tolperisone for 3 days 2 times each day.

Went to doctor after 2 days and took Arachitol 6L Injection.
Also took Limcee tablet for a day. Had Acidity with Limcee and stopped that.
Took 2 tablet of FLAX 369 for 2 days , one tablet each day.

After 7 days took one tablet of Uprise 60000.
After 2 hours of taking the tablet ,there was tightness in the jaw movement.

After 7 days took one more tablet of Uprise 60000.
There was slowness in walking and there was heaviness in right leg.

Took one more Injection of Arachitol 6L after 20 days.
I then had still more difficulty in climbing stairs and not able to climb. I could walk slowly. Right leg , I could not lift up. There was heaviness in leg.

Observation : Before Arachitol 6L Injection , I was walking well and climbing the stairs. After injection and Uprise 60000 I was having difficulty in climbing stairs and was not able to climb a single stair.
After Medical Blood Test when the Vitamin Deficiency was found I was very well walking and could very well climb stairs. After the Injection of Arachitol and UPRISE 60000, I was not able to lift her leg for a single stair and not even can cross a toy on the floor although walking slowly. When there was spasm found or Deficiency found , I was well in walking.
Weight : 39.5 KG
Weight 2 months before : 45 KG
AGE : 30

Kindly please advise on the below.
1) How would I be able to walk and climb stairs? Why I am not able to climb stair. It is due to toxic effect of Vitamin D. If it is then what would be the remedy for the same. What should be the right dose of Vitamin D.Had I exceeded in Vitamin D Intake through 2 Injections ( 6L) and 3 Vitamin D TABLETS ( 60000).
2) What could be remedy for Spasm?
3) What could be Calcium and other elements / medicines needed for me by which I could be cured.
4) I am not having proper Motion and have mild pain on the upper of right side of stomach when pressed. What would be the medication for this. It also looks like something is stuck in throat. What could be the food to take. My weight is very less now and food is not helping me to gain weight.
Kindly please advise regarding stomach and throat also and the medication that can cure me , help me to climb stairs , walk well , eat well as there is still difficulty in swallowing food and still there is restricted neck movement. Sometime suddenly feels like vomiting and cannot take solid food like apple.
In the morning today was feeling like vomiting and took one EMESET -4 , and now have much more difficulty in eating food and much more restricted neck. There is heaviness in chest , and stomach bloating and feels like pulling down.

Kindly please help me to get well.

Brief Answer:
Needs investigations.

Detailed Answer:
Hello,
Thanks for posting your query.
1. The inability to climb stairs is not due to a side effect of Vitamin D3. This is more likely to be due to a muscle weakness. The sudden weight loss that you are having is also worrisome and needs to be investigated. You need a comprehensive evaluation by a neurologist along with MRI scan of the spine and legs to find out the cause.
You are getting the right dose of Vitamin D3.
2. Oral muscle relaxants and cervical traction can help relieve your spasm. You can consult a proficient physiotherapist for this.
3. Proper treatment depends on the diagnosis. If there is any nerve root compression at neck then that needs to be corrected first. You can start with calcium supplements to help you.
4. For constipation you can take oral laxative like Cremaffin syrup at bedtime. The abdominal pain needs to be evaluated with an ultrasound scan to rule out gall bladder calculi.
It is very likely that you have a moderate case of gastritis which is caused by excessive acid secretion and irritation of the lining of the stomach.

This causes a variety of symptoms including pain in the abdomen, nausea and vomiting. Avoid oily and spicy food. Also reduce tea, coffee, alcohol , smoking.Drink at least 2 litres of water everyday. Eat dinner 2 hrs before sleeping . Proton pump inhibitors such as pantop can help you if taken for few weeks. Activated charcoal will reduce the bloating sensation.

I hope this answers your query.
In case you have additional questions or doubts, you can forward them to me, and I shall be glad to help you out.

Brief Answer:
Dose detailed below.

Detailed Answer:
Hello.
Thanks for writing again.
The recommended dose of vitamin D3 in case of deficiency is 60000 IU per week. You can take 8000 IU per day without any signs of toxicity. The doses that you have taken are not excess. You can take at least 1000 mg of elemental calcium daily along with this.
The symptoms that you have are not only due to these deficiencies. You need a more detailed evaluation and MRI scans.
Hope my answer is helpful.

Sir
Thanks for the statistics.

I had taken 6 lac IU 2 injections and tables making 13.8 lacs IU in 34 days making around 40 thousands IU per day.

Kindly advise again as it looks to be toxic as it is more than recommend limit suggested by you.

MRI scan of cervical was done and was normal.

Kindly advise if it is vitamin d toxicity and next step that I should take.

Thanks XXXXXXX

Brief Answer:
Not toxic doses.

Detailed Answer:
Taking 40,000 international units (IU) a day of vitamin D for several months has been shown to cause toxicity. Its raised levels cause hypercalcaemia- increased calcium levels in the blood. You can get your serum calcium levels evaluated again to confirm. The doses you have taken are not toxic.
Regards.
