Question: Hello,
I had posted a previous question last month and it was helpful but I'm still having issues.
Previous issues : Oct. 27th - emerge for pain and swelling starting in the left breast, between the armpit and lungs, by midnight my whole left side of my body swelled.
blood pressure - 183/126 (seriously)
x-ray- nothing
mammogram - haven't heard yet
Nov. 7th - I went to a walk in clinic the following Friday, the swelling worsened and travelled to my right side, my whole body was swollen with fluid, pain was still present as well, in both breasts, and lower back. she sent me for blood work, urine and put me on a pill called HYDROCHLOROTHIAZID (its working very well)
blood pressure 164/110
Nov. 17th - I went back to the same clinic because they called that day and had my results for the blood work but I was also experiencing symptoms of a UTI, including BLOOD CLOTS. He put me on an anti-biotic and (I was told this was the best clinic for doctors to follow up and are thurow, since my family doctor is in another town. I haven't yet received a family doctor here).

blood work - (I'll just give you what's HI)
platelet count 412 HI
absolute - NEUTS 5.5, LYMPH 2.6, MONO 0.8, EOS 0.8 (HI), BASO 0.1
under the urine section - appearance - cloudy
specific gravity - 1.030
pH 5.5
blood small HI
creatinine 91
eGFR 59
sodium 139
creatine kinase (CK) 274 HI
So, my concerns...my blood work, the doctors went threw it with me and said what was HI but also followed it with, "It's not a concern, it's not a concern ect., " I told him I was very concern about the CK level because it August 2013, it was the SAME!! The blood clots also really scared me. The doctor I had in 2013, did follow up blood work, told me the concerns with kidney disease although scary but I worked on it and after 6 months (January 2014) my CK was down. He also gave me an anti-biotic, sulfamethoxazole. I was not impressed with medical help I'm receiving and I cannot afford to go see me family doctor, besides where I live there are no specialists. I finished it last Sunday but my back is still very sore, I wish I could see someone here or get some consistant help from a doctor here, I am very worried I may have a kidney problem. (sorry so long)

I appreciate your concern

I would like to address each issue one by one.

1) The blood pressure readings are quite high and need to be monitored at home thrice a day for a month at least , to look for persistent elevation, in the same range. A 24 hour ambulatory B.P monitor could give a more accurate assessment and you may ask for it if available in the hospital in your area.

thiazide diuretics like hydrochlorathiazide are weak diuretics and weak anti hypertensive medication. If the B.P is persistently elevated then an ACE inhibitor can be added to the diuretic.

2) I would like to know the units of the above reports. could you please quote the reports along with their units or you may upload the document of the report itself.
the elevated eosinophil count can be from allergies, worm and parasitic infestations, drug allergies etc.

3) GFR Levels below 60 mL/min/1.73 m2 for a period of 3 or more months are a sign of chronic kidney disease and you should repeat the tests in 2 months time as its slightly borderline and may be normal for a thin person or vary due to certain drugs as some nephrotoxic drugs can lower GFR and raise creatinine. Again the creatinine value given is not very clear and doesn't match the GFR. Please upload the original document if its possible for these tests , too.

4) CK normal range is 60-174 IU/L. An elevated level can indicate
Myocardial infarction ,Myositis ,Myocarditis (inflammation of the heart muscle) certain Medications (like Statins which are cholesterol lowering medication,)Hypothyroidism and renal failure can also present with an elevation

So there are a bunch of causes, but a cardiac cause must be ruled out first. An EKG should be done , if clear then other causes should be looked for.

5) The platelet count is slightly elevated and is considered normal, you shouldn't worry much about it.

6) Since the empiric antibiotics didn't treat the UTI, you may ask your doctor for a culture and sensitivity testing of urine to target the specific bugs responsible for the infection. You may also get an Ultrasound abdomen Pelvis done to rule out any other cause eg a stone or any gynecological issue. You may discuss these options with your doctor.
