What Causes High Heart Rate Despite Having Normal ECG?

Question: Hello
I would like a second opinion
I am a 43 ur old women who is fairly fit in Thursday I had quite a scare and after the fact I noticed my heart rate was high so I went to triage they gave me Ativan and slowly that and blood pressure went down they did traponin test that came back normal
They did find my pottasium low
I did ECG - that came back normal but fast heart rate
Echo - they said it is fine
I am not over weight
I work out 3x a week
I do suffer from anxiety and panic
I would like a second opinion if this is cardiac related
I am currently warring a loop monitor and it has gone off on its own does this mean something bad also I am flying out for vacation next week
I can upload my results of blood and ECG

I carefully reviewed your uploaded ECG (there are no blood lab tests, just 3 uploaded ECG, which seem to be the same) and would explain that they are perfectly normal. They just indicate sinus tachycardia, which means a higher heart rate in a healthy individual.

There are no signs of possible myocardiac ischemia or conductance disorder.

Besides, there are no evidence of any structural abnormalities in your cardiac ultrasound, which is another argument that shows that your heart is perfectly normal.

All this clinical situation seems to be caused by a panic attack or anxiety. The fact that ativan helped you recover from this situation is another strong argument that all this situation was caused by pure anxiety.

So relax and do not worry about it any more! You should try not to think about it!

Regarding the loop monitor it is quite normal. You should not worry about it.

Coming to this point, considering all your symptoms and performed tests, I would conclude that there is not counter-indication for you to go on vacation next week.

So just try to relax! Everything is going to be OK. I can assure you about that!

The only thing I would recommend is consulting with a psychiatrist in order to optimize your anxiety therapy (ativan is just a short term therapy, and an antidepressant could be more helpful in preventing and managing possible similar episodes in the future).

One last question about the holtet monitor I notice it has gone off several times without me pushing the button obviously it is picking something up can this be the tacardia because sometimes when it beeps I don't have any symptoms

You don’t have to worry about Holter monitoring device.

It keeps registering every rhythm events without any warnings. The button you may push just helps the analyzer to know the moments you have experienced any unpleasant symptomatology, but it does not interfere with continuous ECG registering.

So, relax and don’t worry about intermittent beeping as it depends on the device settings and specific programming.

Tachycardia is just one of the heart rate variations; and there is no reason why Holter monitoring would work out of its programmed regimen.

There are devices that may beep while tachycardia is present, others no. it depends on programming.

I think everything is OK and the most important is the rhythm conclusion, not just the process of warning while registering.

Thank you so much with ur responsed and patience as u can see this is making me anxious and I really want to go on my vacation next week just afraid they will say I cannot fly

Hi there
I tried that link u provided but it didn't work
I just found out this is an event monitor also if the results come back that I have tacardia or pvc can I still fly out next week I should have all the answers by Friday
Just wondering if I can still fly again

Anyway, I would explain that an event monitor records your heart's electrical activity only at certain times while you're wearing it. This means that it is not like Holter monitoring which records during all the time. That is why it goes off time after time. It gets on again when your heart rhythm begins to have any abnormalities (cardiac tachycardia, ectopic heart beats, etc.).

Based on your symptoms and your other performed cardiac tests which hace resulted normal, I would conclude that your symptoms are not related to any cardiac disorder.

Besides anxiety, low potassium levels are another contributor to your symptomatology. You should know that potassium plays a very important role in your heart rhythm. Low potassium levels can induce cardiac arrhythmia.

That is why I recommend taking a diet rich in potassium, in order to correct your potassium levels and prevent possible cardiac arrhythmia related to low potassium levels.

I would explain that ectopic heart beats caused by low potassium levels can trigger a panic attack and the situation can worsen itself (like a circulus vitious).

So, it is important avoiding this triggering factor.

I am sure you will be OK for your vacation! There is nothing wrong with you!

I have just had my event monitor go off with a long beeping sound this has not happened before during my time wearing it

I would like to explain that it has been activated probably by tachycardia caused by a physical activity or anxiety.

You should know that your anxiety is activating it more and more, because as I explained before, emotional stress and panic attacks can lead to tachycardia and even ectopic heart beats.

The only thing you should do is to close your eyes, take deep breaths, think about nothing and try to relax.

You should focus on your breathing with eyes closed. This will help you calm down.

If this does not work, I would recommend taking 1 tab Ativan. I recommend you to write down the time of ativan intake and show it to your attending physician, when your monitor results will be interpreted, in order to come into correct conclusions.
