What Causes High Fever In A Child?

Question: I need to talk to a pediatrician regarding my daughter's health condition. She has had a high grade fever for 2 days (wed+thu) which even after giving crocin syrup and ibulgesic woould come down only after abut 4 hours or so, and since last night it is less than 100 degree celsius which we have been able to control with crocin syup.

Welcome to the HCM

I have gone through your question and reports attached. I completely understand your concerns. The symptoms and her reports are suggestive of bacterial infection most likely due to enteric fever or typhoid. Increase in Monocytes with normal total leukocyte counts and increased CRP are in its favour. Don't worry as she is already responding and fever is coming down.

I would recommend Typhidot IgM and blood culture and sensitivity test to confirm the diagnosis.

You may continue with the same medications. Zifi is useful to treat typhoid as well but kindly reduce the dose to 20 mg/kg/day ( maximum 400 mg/day) in 2 divided doses. Also, start her on probiotics such as enterogermina with oral rehydration therapy for diarrhoea. Give her regular small feeds.

How do I give her enterogermina? Also she hasn't had fever since this morning - still this could be Typhoid?

Hi

We give enterogermina sachets or ampoules twice daily.

Although most common reason for such episodes in this age group kids are self limiting viral fevers, they have normal blood picture in most instances.

Typhoid fever is a possible cause of fever in your daughter because the CRP is high and Monocytes are increased with normal total counts. Zifi which is an antibiotic acts in many infections including typhoid. Since you had started Zifi on the very 1st day of illness and fever responded after around 2 days, typhoid fever cannot be excluded without further evaluation. So, I would recommend typhidot IgM and blood culture and sensitivity.

If positive, then you will have to continue antibiotics for10-14 days for complete treatment. Else, you may stop zifi after 5 days of completion if she is asymptomatic.

In case she develops any other symptoms or signs, then you can inform us. I would be glad to help you out.

We actually started zifi last night which was the 2nd day of the fever .. She had diarrhea twice post that and low grade fever around 2.30 am (99.5)which went away after crocin. She had fever again this moring at 9.30 am (99.9) which again went away after crocin. Post that no fever again but she vomitted after hving her ORS and then she passed watery stools thrice.

After how many dayscn we do the typhoid test?

If the fever responded after just 1-2 doses of zifi, then the possibility of typhoid is less likely. Because any antibiotic therapy takes 24-48 hours to bring down the fever spikes in typhoid.

So, I would recommend you to wait and watch the pattern of fever spikes. If they are gradually improving, then it's unlikely due to typhoid. Do not go for any further tests in that case.

You may continue the same treatment. In case, there is any other symptoms in future then do share it so that I may help you out.

Hi,

We did the Typhidot test using the same blood sample that was drawn for the CRP etc.. The results have come out negative. She has had no fever since yesterday but has been having diarrhea which is dark green in color and slimy .. seems like she passes stools each time eats or drinks something and due to the fear of passing stools again she doesnt seem to want to eat anything. Has eaten 1.5 Vita XXXXXXX biscuit and a glass of apple juice which she has managed to retain so far.

Is it Zifi that is causing the diarrhea or is it something else we should be worried about? She has no fever and has been playing and running around the house as before but I am worried she will get weak if she continues to have diarrhea and doesnt eat anything.

What are the foods she is allowed to eat?? My husband and his mom seem to think she needs chicken and foods that will strengthen and I think chicken is too strong for her ..at the moment. your expert advice please.

Hi

Do not worry. Any gastroenteritis episode takes usually 3-5 days to improve in kids.

Enterogermina which I suggested will take care of any zifi associated diarrhoea.
Other reason can be infection or worm infestation. As the stools are slimy and green with urge after each oral intake, I would recommend you to start her on Syp. Ornidazole ( 20 mg/kg/day) in 2 divided doses for 3 days. This drug helps clear any amoebic or giardia infestation leading to such symptoms.

Also, start a zinc supplement such as Syp ZnD or Zinconia to help in early recovery. Avoid sweet juices as they may aggravate toddlers diarrhoea.

You may continue oral rehydration therapy such as lemonade, buttermilk, soups, curd, etc. Chicken soups and well cooked chicken are fine if she wants. Give her regular small feeds.
