What Causes High Blood Pressure Despite Taking Nebicard?

Question: By profession I'm an academician (40 years old, male), teaching B Tech, M Tech, M Sc students, as well as guiding few Ph D scholars, therefore come across tremendous work pressure and suffer from hypertension and therefore often stressed out. Couple of years ago I have undergone ECG, TMT and all blood works- KFT, LFT, lipid profile etc. only abnormalities that could be seen was that my cholesterol level was slightly higher than normal, in TMT I was diagnosed ischemic, but six months later when TMT was repeated, it was fine, I was prescribed with an antacid, blood pressure, aspirin-atorvastatin and a mild doze of alprazolam. Now I'm continuing with the antacid (esomeprazole- 40 & now 20 mg) and blood pressure (nebicard 2.5) medications for the last 6 months. Recently, I am finding a problem that sometime I find lightheadedness and higher pulse rate than normal, often in night times and if I stop antacid for couple of days, I get into acidity problem. With a sudden check last night after a busy work day I find my blood pressure 147/90 and this morning it is 110/73. What should I do?

Hi,

I understand your medical history and glad you gave details.

With what I understand you seem to have hectic work schedule and the current medication nebicard 2.5 mg is not able to control the evening blood pressure.

In such a case I suggest my patients to take a dose of antihypertension medication at night and check the change after two weeks. It is important to keep BP recordings thrice a day and show it to the doctor called 'BP Diary'. The doctor can titrate the doses and give the best working BP medication (there are many anti BP medications that work differently). The goal should be to keep the blood pressure under 140/90 anytime with medication and life style changes. So please go back to your doctor about my suggestion and necessary modification.

One extremely important thing is to modify lifestyle with at least 3 hours of exercise a week and increase fibre (with 5 cups of fruit and veg per day). I suggest to reduce the intake of white rice, white bread, sugar and salt in your daily diet.
