What Causes High HbA1C?

Question: HELLO DOCTOR,I AM 52 YEARS OLD FEMALE ,I AM ON MEDICATION FOR DIABETES ALONG WITH BLOOD PRESSURE AND THYROID MORE THAN 12 YEARS.FOR DIABETES MY DOCTOR PRESCRIBED ME JANUMET 50MG/1000MG,PINOM 20 FOR BLOOD PRESSURE AND THYRONORM FOR THYROID...AS I HAD CHECK MY SUGAR LEVEL FEW DAYS BACK..RESULT WAS FASTING :- 211 AND PP:-300 .PLEASE KINDLY SUGGEST IF THERE IS ANY KIND OF CHANGE IN PRESENT DOSAGE OR MEDICINE CHANGE REQIURED...I AM ALSO ATTACHING MY REPORT

Brief Answer: Some more information please. Detailed Answer: Hi Mrs XXXXXXX XXXXXXX This is Dr XXXXXXX physician and expert Diabetologist answering you. The medicines you are using are no doubt very good ones. But let me ask you few questions to understand why your diabetes is out of control. 1. How many tabs of janumet are you taking per day 2. What is your height and weight ( atleast roughly ) 3. Do you walk or exercise dialy ? 4. What is your regular diet. Please understand that all the 4 questions are extremely important for giving you correct suggestions. thank you.

I TAKE 1 TAB TWICE DAILY,MY HEIGHT IS AROUND 166CMS AND WEIGHT 84 KGS,YES I WALK DAILY HALF N HOUR IN EVENING..1 CHAPATI WITH VEG MEAL WITH 1 CUP MILK IN MORNING,RICE WITH DAL ,FISH CURRY IN LUNCH,AND AGAIN RICE WITH MIX VEG OR SOMETIMES NON VEG IN DINNER..ALSO TAKE SUGARFREE TEA TWICE..

Brief Answer: The best option to you is insulin Detailed Answer: Fine, Your diet and exercise is fine. With the presumption that you have high HbA1C and poorly controlled sugars since sometime, i believe you have two options: 1. The best option to you is insulin. 2. If reluctant to use insulin for otherwise reasons, you may need an addition of a sulfonyluria tablet like Glimeperide which may improve your fasting as well as pp sugars. (less likely) As you are obese (your BMI IS >30) and already on 2 grams of metformin and 100 mg of gliptan; with a healthy life style, which is still not really working, I would recommend insulin for you. Insulin offers you high quality of life with best achievement in glycemic control, especially the newer generation insulin pens, which can be used with ease. Next time you visit your doctor, make sure that you are well prepared for it. All the best.
