Brief Answer:
I would explain as follows:

Detailed Answer:

Hello,

I understand your concern and would explain that your symptoms could be related to heart failure.

Besides, your blood pressure values are high, which means that you need to start antihypertensive therapy besides diuretic.

Coming to this point, I recommend consulting with a cardiologist and performing a cardiac ultrasound, a resting ECG and NT pro-BNP levels.

Some blood lab tests (complete blood count, blood electrolytes, fasting glucose, kidney and liver function tests) are necessary too.

Optimization of your current therapy is necessary to improve your situation (increasing diuretic doses, ad an ACEI like lisinopril or ramipril, etc.).

Hope to have been helpful to you!

Regards,
