    What causes heart burn in a diabetic patient?

Question: I am 60-year old female with Type I diabetes for 52 years. I have what feels like major heartburn the entire length of my chest. But no reason for heartburn, since I have only had a handful of raw almonds since noon today. My glucose has been over 300 since mid-morning when I changed and refilled my insulin pump. Can't seem to get it down. Am extremely tired. Do I need to consider this to be some kind of heart event?

Hi,
I am Dr. Prakash H M and I would like to answer your query.
When I see am elderly patient with a major heart burn, I seriously consider two issues
1.     Possible cardiovascular event
2.     Gastritis / peptic ulcer disease
However, until I rule out the first event, I would not think about considering the second event. Diabetes has been known to increase the risk of cardiovascular disorders and most often silent myocardial infarction (Painless heart attack).
As it is a life threatening event, and each second lost can lead to major and irreversible heart damage, I would suggest you to go to the Emergency department in the hospital and get an ECG done. Other tests for cardiac enzymes like creatinine kinase, LDH and troponin study are also essential and can be done based on the ECG findings.
If it is a peptic ulcer disease, It wont be a major issue compared to a possible cardiovascular event. Drugs group like proton pump inhibitors like omeprazole and histamine receptor blockers like ranitidine can be used. You can try some anatacids too for immediate recovery.
Go to the Hospital emergency are once / give a call to your doctor at once.

If it is not a cardiovascular event, we can discuss the other possible causes later.
