What Causes Headache, Floaters In Vision And Blurred Vision?

Question: Today before lunch I was at the office. I started getting a fuzzy large floater in my vision. The floater (one towards the right) wasn’t black but rather almost kind of a squiggly light. It lasted about 30 minutes. I left the office and went to lunch. While at lunch I had a slight headache. I don’t ever get headaches but I wasn’t concerned because yesterday I received Botox and figured the floaters and the headache were caused from the injections. In the middle of lunch my friend was asking me if I knew a certain person. She addressed this person by name. I know the person very well because I work with them every day, however I knew in my mind I knew this person but couldn’t remember who it was...just that I knew I worked with them. After that, I went to ask her if she knew someone and when I got to the point where I would say the name, I couldn’t remember it. Again, this is someone I know very well but couldn’t remember their name. Within five minutes, I remembered everyone and went back to normal. Should I be concerned?
