What Causes Headache And Nausea After Taking Rifampicin And Isoniazid?
Question: Dear doctor, this is XXXX again regarding my wife's latent TB treatment queries. I hope you don't mind few more questions.
You might remember that when my wife took Rifampicin + Isoniazid she developed headache, nausea after two weeks so Dr stopped medicines for 10 days. then Dr started only Isoniazid. Again after taking Isoniazid for two weeks she developed rashes. Her rashes have cleared now and Dr stopped Isoniazid and started Rifampicin 600mg only. After taking Rifampicin for one day she had minor headache. She is going to see how she feels next 3-4 days.
1> If she can not tolerate Rifampicin as well, then what should be done to treat Latent TB?
2> Are there any other option apart from Rifampicin and Isoniazid for latent TB treatment?
3> If Rifampicin and Isoniazid do't work for her then is it better to stop latent TB treatment now and carry on routine life?
4> I noticed that her CPR (C-Reactive Protein) reading was 38 last week and month ago it was 52. Do you think it is too high? She has not any fever or other symptoms. Her BMI (Body Mass Index) is 31.
Please answer with your experience and any possible replacement medicine names or treatment names.

1.She should be able to tolerate Rifampicin as so far she has only minor headache. Here let me tell you , most of the patients taking tb drugs have some unpleasant side effects to them. But these generally pass of as patient continues the treatment. So do not get unnecessarily perturbed with minor side effects. However skin reaction and hepatitis are serious side effects and cannot be ignored.
2. Yes there are other tb drugs which can be given like etambutol and other second line anti tb drugs. But these cannot be taken as over the counter drugs. Expert supervision of a pulmonologist is mandatory. So let her treating physician decide the regime for her.
3. Yes that is another option for her. Since she does not have active tb, you can stop the tb treatment chances are she may never develop active tb. In her case latent tb is being treated to enhance the success rate of IVF.
4. CRP is a nonspecific marker and do not attach too much significance to it. It may have risen because of medicines that she is taking.
