Question: Hi Doctor,

I'm an investment banker and my job involves long sitting hours. I have being eating lots of peanuts since the start of winter and it caused (I believe) hardening of stool, three days back It got so hard that it ruptured some tissues during passing the stool in morning and caused bleeding. Since then I have been having this pain which subsides in morning when I get up but during the day when I relieve stool it starts again and keep hurting the anal area. I applied some almond oil at affected area, it helped during the night but again same situation during the day. I have started taking high fiber and lots of water but this pain and muscle swelling hasn't gone from three days. Kindly help doctor. I'm living in XXXXXXX XXXXXXX

I have read your query and understand your problem very well.
Your history suggests that you are most probably suffering from anal fissure.
Anal fissure is a painful tear in your inner Lining of anal canal and is generally caused by the passage of hard stool during the bowel movement.
Treatment involves...
Relieving of constipation by taking high Fiber diet,drinking plenty of water,doing regular walk for 40-50 minutes a day and taking some laxative like lactulose or CREMAFFIN syrup in the night.
Applying local anesthetic ointment like lidocaine or xylocaine.
Taking anti inflammatories pain killers like ibuprofen 400 mg three times a day.
      However,
You must consult a surgeon to get your condition assessed and make the proper diagnosis as sometimes other conditions like hemorrhoids should be ruled out.

Thanks for your suggests Doctor, I would follow all plus medicines as well. I checked with nearby pharmacist here and they all have LOX 2% Jelly (LIGNOCAINE HYDEROCHLORIDE GEL I.P), they said lidocaine or xylocaine are no more available. Shall i go ahead with this LOX 2% Jelly?
Also I'm having black chickpeas, Arhar lentil, baazra roti, green chickpeas (fresh), carrots, Mooli, white eggs in food and in fruits I'm taking Oranges, Pomegranates and Chicku everyday in good quantity, hope it's fine or you suggest anything else should I start eating (or rather stop).

Hello,
LOX jelly is all right to use.
You are eating fine.
Take CREMAFFIN plus syrup 3 tsf in the night for 7-10 days to make your stool loose so that the tear is healed,however as I suggested above,you must consult a surgeon for a proper physical examination.
Use ISABGOL husk 15 gm daily with 250 ml of water for longterm use to take care of your constipation.
Start using ISABGOL after you stop using CREMAFFIN.
