What Causes Fluid-filled Blisters On Penis?

Question: Hi, my brother has this fluid-filled blisters on his penis cap. He has done a urine and semen M/C/S with growth in the culture after 48hrs. The pus cells in both specimen is 1-3/HPF and the Epithelial cells is 1-3/HPF. Crystals in urine is 2-4/HPF. And Bact cells in urine +.
What does the above mean? Does he need to run more tests?

Hi,

I have gone through the medical history and lab reports of your brother and I can say as following:

1. The lab reports of both urine and semen microscopy and culture are within normal parameters, and non indicative for respective infection.

2. You are saying that he has a fluid-filled blisters on his penis cap. Can you please provide further information at this point:
- a photo of the blisters
- if he is having pain? burning sensation? itching?
- history of unprotected sex (either oral/vaginal/anal)?
- is he circumcised?

3. You are saying that he was taking Orelox for sore throat at the time when lab analysis were carried out. Please take note that Orelox is the trade name for the antibiotic Cefpodoxime (cephalosporin). I'm afraid that this antibiotic has altered the lab results giving a possible false negative results for infection.

I usually recommend my patients to take the lab analysis as your brother did at least 1 week after finishing the antibiotic treatment and not during the treatment to avoid false results.

Yes, he experiences a dull pain when he presses the cap. No itching. He is circumcised. And he's had unprotected sex (Oral & Vaginal) recently. He applied Gentian Violet to it after the blisters burst and it's dried up and he's not feeling the pain anymore.

Please how do we upload the picture of the penile blister as it is this moment?

I have uploaded the pictures as requested. Please bear in mind that this is the most recent pictures. And the GV has been applied as that moment.

Hi back,

Thank you for providing the requested photos and information.

From what you are reporting, the penile blister he is having has nothing to do with an internal problem (urinary-genital infection) as per the lab reports.

It might be due to a fungal skin infection, which is being improved by using the Gentian violet solution which is an anti-fungal XXXXXXX

Hopefully, with the aid of gentian violet applied locally the blister will be fully recovered. Orelox has also fight any possible bacterial co-infection.

If it will not be subsided, then, a further lab testing of affected skin scrapping under microscopy is advised to determine the right diagnosis.
If no improvement, a further systemic antifungal medication can be used (like fluconazole) together with a topical cream combining nystatin (anti-fungal) with neomycin (anti-bacterial) and clobetasol (anti-inflammatory) medications.

Hi,
I want to add that the first time it appeared he took ampiclox and also applied Gentian Violet. This made it to dry up and he completed the ampiclox medication. Then it appeared again about 8weeks later.
Can he go for the skin scrapping or should he start the antifungal medication?

Hi back,

As this has been repeated in 8 weeks, then, I advise to run the skin lab testing to understand if it is bacterial or fungal and get it treated correctly.
Before you go for lab test, he needs to stop using Gentian violet locally and go for test when it is completely de-colored (washed the XXXXXXX completely).

You can discuss this further with his dermatologist.
