What Causes Fibrocalcific Densities And Air Trapping In Lungs?

default
Posted on Sat, 27 Sep 2014
Question: I had a CT of chest (HRCT ) done and some of the issues highlighted are :
1. Fibrocalcific densities are noted in posterior segment of right upper lobe sequelae of old granule atoms exposure.
2. Areas of air trapping are seen in both lung fields, more so in the posterior segments of upper lobes and basal segments of lower lobes- small airway disease.
3. There is a focal bulge seen upper pole of visualized left kidney. I already got a USG done with no such comment.
4. Bones in view show degenerative changes with both anterior and posterior osteophytes more prominent in lower thoracic upper lumber region.

Can you kindly explain this in simple lay man language for better understanding.
Thanks

Detailed explanation is given below please

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail and would like to explain your CT findings in simple language.

1. Fibrocalcific densities are noted in posterior segment of right upper lobe sequelae of old granule atoms exposure.

This means there are few small calcifications, probably the size of mustard grains in your lungs, in right upper lobe. These can be due to a very old lung infection which might have occurred many years ago. Please do not worry about this finding.

2. Areas of air trapping are seen in both lung fields, more so in the posterior segments of upper lobes and basal segments of lower lobes- small airway disease.

When CT scan is done, the patient lies on the bed and there is always little amount of air trapped in the lung air spaces. This is well seen in elderly at your age. This is seen in posterior segments of upper lobes and basal segments of lower lobes and is consistent with changes in lung appearance with age. The mention of small airway disease is used at younger age group based on the exact pattern of involvement which needs to be seen. At older age, this can be more obvious if patient is over weight and obese.

3. There is a focal bulge seen upper pole of visualized left kidney. I already got a USG done with no such comment.

This finding needs to be confirmed by seeing the picture and doing ultrasound. As your ultrasound is not mentioning about this focal bulge in left kidney upper pole, I assume it was a normal appearance and not disease condition.

4. Bones in view show degenerative changes with both anterior and posterior osteophytes more prominent in lower thoracic upper lumbar region.

These are concerning age related degenerative changes in your spine. This is expected at age 60 years and is due to regular wear and tear of the spine.

Hope your query is answered.
Do write back if you have any doubts.

Dr.Vivek
