Question: I have been experiencing extreme exhaustion for the past 5 days, including inability to go to work. Sudden onset, with intermittent headaches, nausea, and some leg cramping. I feel hot, but not "hot flashes," and no fever. Physical one month ago was normal, including thyroid (have hypothyroid, take .88mc sythroid)

Welcome to HCM and thanks for the consult. I have thoroughly reviewed your health query and can well understand the sensitivity of issues especially inability to go to work.

There are many possibilities that may contribute to the issues. Vitamin D3 and Vitamin B12 levels are needed to repeat afresh in order to see the status of hormones, effectiveness of vitamin D3 therapy and to see if vitamin B12 is also low.

Thyroid function tests are needed to see if there is any need for change of medicines being given for your Hypothyroidism. Sometimes there is a need to re-adjust the thyroid medications time to time. Consult an Endocrinologist for the same.

Liver function tests are needed to see if everything is fine with liver and gallbladder and their circulatory fluids including bile. Once verified through radiologic and blood test studies we can comment on that.

A lot of your symptoms fit into hormonal imbalances very common at this age. Your physician after evaluation can suggest if you need hormonal replacement therapy or not.

Blood sugar levels done at walk-in clinics are random levels (we ca do anytime throughout the day). For proper diagnosis of Diabetes Mellitus better is to screen the patients with Fasting blood sugar and HbA1c levels. Clinical history of patient is also important in this regard.

Take care of your diet too I must add here. You need to keep yourself hydrated well in order to remain active and feel better.
