Question: I am get so exhausted that I can't function. I have not been able to do anything today. I get pale, nearly lethargic feeling, yawn a lot. If I lay back in my recliner to out feet up ad rest it gets way worse. It seems to get worse because of my feet being inclined in recliner. I get sometime that I fight to wake up and get out of the chair but just can't. It is like a dream yet is real. I do suffer from some depression and don't always boil over with energy, however these tired spells seem to come in waves. I have been to Dr and had tests run more than once. Thyroid and Iron have been check more than once. My Dr. did run a mono test last year and it showed that I had it in possible recent months or even longer ago but was not active and I think she said it showed immunity meaning I'd had it int the past. When it gets bad like today I feel almost drugged when trying to make myself wake up or get around and do things. I have noticed in the last year my words seem to get tangled when I am trying to talk, I say them backwards or say a different word that I even intended. It often is one that doesn't make sense when uses with what I was saying. This is not like me as I have always been very clear thinking and speaking. Very prompt, and enunciated. I was diag. with fybromyalga yrs ago. Also my blood pressure will run very low at time. The lower number will get down to 50's and even 40's. I use a digital wrist one at home. It doesn't always run this low but goes it spells and will nearly always run low if I lay back in chair and raise my feet. I get sleepy and tired fast if I do that. Some of the readings in memory are 104/59, 106/58, 118/63, 94/44,
97/54, 99/54, 98/67, 105/61, pulse 57, 91/50, 107/58, 96/55 pulse 64, 100/56 pulse 62. It will run a bit more normal at times 120/65, 110/62, 121/69, 130/74, 108/71, 118/70.

Thanks for asking HCM.
I am Dr Rakesh Sharma answering your query.
You have not mentioned about you daily routine,diet, exercise,and when you saw your doctor last time or when doses of your drug modified, would have given some more idea.
Any way from your information the conclusion I could draw is you may be suffering from postural hypotension with which is low blood pressure when you change your posture. There are plenty causes of this but the one suited most to you, it may be the antidepressant, you are taking is known to cause low blood pressure though in very very few cases,and may be the depression still existing even after medication.
Have diet full of protein , fiber and carbohydrate if not a diabetic, you can see a dietitian for the same. Drink plenty of oral fluids.
Mild Exercise,what ever your body can sustain and yoga are supposed to be very helpful in these type of cases.
You can also see your doctor to review the dose of your drugs, if he wants to modify or change it as per your complaints.

I have been to my doctor in past 2-3 months about this. I did have issues with it before the Wellbutrin was started. We started that because we thought some of the exhaustion may be due to my untreated depression. The Wellbutrin helped somewhat with the mental energy for a while but I still battle the physical tiredness fat times. As far as routine, we get up at 6am and turn in about 11pm. I don't get as much exercise as I was due to the current exhaustion spell I am having. We have not changed any med doses. I take 100mg once a day. My diet could be better. My sister and mom are bad diabetics, my dad was border line and my niece has hypo glos. I have tested my blood sugar when I am lethargic feeling and it seems to be normal. in the 90's. I can eat a full meal, for example pork chop, veggie, something sweet with it and the two hour peak was 105. It never goes to high even when eating things. I am 5'1" and weigh 135. I am just sitting in the chair replying to you, and BP is 104/66, pulse 69. I have been up and doing things around the house most of the day and just sat to do this. I just ate a sandwich about 30 minutes ago and just finished a glass of tea.

It is good to hear that you do not have high blood sugar levels.
All your symptoms: 1. exhaustion, 2. Fluctuating low blood pressure and 3. Low pulse rate. All these complaints do indicate to see a heart specialist to rule out cardiac aliment.
And also as stated in my previous answer in consultation with your doctor to modify dosage or change your medications (antidepressants or HRT)
Apart from exercise, you are doing most things as required, but I would still suggest you for the same as exercise as much as you can and yoga will also increase oxygen inflow in your body which will make you feel little better.
Mean while chocolates and few cups of coffee is another helpful drink for your low mood and low blood pressure.
