Question: Having extreme fatigue combined with chest pain weakness occasionally dizzy upon standing. Just don't feel right. 34 year old construction worker. Father and husband. Very concerned

Welcome and thank you for asking on HCM!

I understand your concern and would e explain that your symptoms seem to be related to orthostatic hypotension, which is a decrease in your blood pressure while standing up.

Although the chest pain, could not be entirely explained by this disorder.

A chronic anemia or a metabolic disorder (thyroid dysfunction, adrenal gland dysfunction) could also cause this symptomatology.

Have you measured your blood pressure lately?

I recommend consultin with your GP for a careful physical examination, a resting ECG, a chest x ray and some blood lab tests:

- complete blood count (for chronic anemia)
- PCR, sedimentation rate (inflammation)
- thyroid hormone levels
- cortisol plasma levels (adrenal gland function)
- fasting glucose
- blood electrolytes
- kidney and liver function tests
- cardiac enzymes (for possible coronary artery disease).

A head up tilt test is necessary to rule in/out possible orthostatic hypotension.

A treadmill test may be needed if suspicions of possible coronary artery disease are raised, during physical examination and resting ECG.

Meanwhile, I would recommend taking plenty of fluids (water, juices) to avoid dehydration and closely monitor your blood pressure.
