Question: My husband is falling asleep during the day while sitting or at work. He immediately begins to mumble, moan, talk, move his hand and feet. He does this multiple times each evening after he gets home from work. He said he does fall asleep at work at his desk sometimes. This began a few months ago. He also falls asleep as soon as he goes to bed and talks, moans, mumbes and moves around for a lot of the night. He has never does this before.

The query is slightly incompelte as it dosent mention if patient get a good night sleep at all or not (does he feel fresh in morning)?

Second,it is important to know if he uses any subsatnce of abuse- like: alcohol or beer or anything else.

3. Had he started any new drug recently ?

4. Is there a recent change in job or any new stressful event in recent past? Or is he anxious these days?

If everything above is fine and there is no apparent reason behind this presentation, then it he should get an MRI scan brain , routine blood tests.

We usually try to give some sedative for few days in starting , like zolpidem 12.5mg: at night. This will help in giving a good and refreshing night sleep. This good sleep will help him stay alert in daytime. Actually daytime sedation is due to insufficient night sleep.

1. He does not sleep well.
2. No substance abuse.
3. Takes no medications.
4. His mother passed away about 5 months ago and he is executor of the will. He says this does not cause him any anxiety. He says he does feel anxious...mainly because he is forgetting things as work (he is an engineer that owns his own business) and he has trouble concentrating.

paeoxetine & zolpidem along with blood tests

Hello,

Most probably its due to anxiety only...may be due to death of mother...its difficult to pinpoint the reason in most cases.
It will be wise to start an anti anxiety medicine. I usually rely on paroxetine 12.5mg at night time. Aling with this a short acting and non addicting sedative should be added for initail 7-10 days.
Zolpidem 10/ 12.5mg at night for sleep is usually sufficient for most people. Later when his anxiety starts reducing, concentration will again become normal and he will not need sedative.

I hope that he should start feeling better in a week of starting treatment. And sleep should also improve in a day or two.

Do get his routine blood tests.

You can get an Mri brain scab to rule out organicity there if you are very anxious.
