What Causes Excessive Around The Anal Region During Bowel Movement?

Question: I understand there may not be a simple answer to my particular problem (not really a problem but more of a nuisance). I have searched on different forums online and have found some people who have posted with similar issues (so I know Im not crazy). I couldn't find anything regarding a solution. My problem has been associated with sweat that occurs before a bowel movement. I typically have 2-3 bowel movements a day. My movements usually occur in the early part of my day. As soon as I awake (around 4-5am) and after my very strenuous workout (approximately 2-3 hrs after I awake). I will then sometimes have a 3rd one later in the morning. What has been a problem for me is the 3rd one. It is typically a much smaller movement and when I get that felling like stool has entered my rectum I begin to perspire down there. This perspiration occurs near my natal cleft and with the cheeks. The only thing that causes this perspiration to stop is having the movement. Even if the movement is extremely small in stool volume. Im not sure if injecting BOTOX in that area is a suitable option. Im not sure if its related to nerves that might be sensitive once stool enters my rectum. I have read that is could possibly be my external sphincter muscle tensing so much that it causing the sweat down there. The sweat will go through my boxer briefs and is sometimes noticeable on my jeans. I have been a bit embarrassed to speak to my gastro regarding this. Any suggestions or advice would be greatly appreciated as Im at my wits end with this.

Hi and thank you for asking on HCM!

I read carefully all your concerns and I can say as follows:

1. I need to know more about your symptoms like :
- when did they started?
- are your symptoms related with particular food, stress, emotions?
- did you use any particular drugs or antibiotics before all this started?
2. To my opinion your symptoms ( sweat before bm) may be related with :

- nervous vagus innervation syndrome. It means that bowel peristalsis starts a vagal response like sweating.
- IBS and probably bacterial gut imbalance.
Otherwise If I were your treating doctor I would suggest to do :
- stool culture test
- sigmoidoscopy
Meantime i suggest to change lifestyle like :
- use fresh vegetables and fruits
- use plenty of water
- avoid unnecessary stress
- use probiotics
- use butifar as IBS treatment.

Thank you for the prompt response. All of the lifestyle changes you have suggested I have already made. Your first thought (Vagal Response) is very interesting and I must look into that. I am positive that when stool enters my sigmoid colon or into my rectum then the sweating begins. It becomes excessive unless move my bowels. Could Botox in this region be a good first step? I know this will only address the symptoms and I would still have to search for the cause. What type of Dr would I speak to regarding my Vagal Nerve and testing it? Not sure any tests exist for proper functioning of the Vagul Nerve. Thank you.

Hi back,

With regards to Botox injection, in clinical practice it is used in anal incontinence and not in such cases like yours.

To my opinion you firstly should perform the exams that I mentioned above. After you have rule out gut issues you should consult neurologist specialist regarding my opinion of vagal response.
