What Causes Episodes Of Tingling In The Arms And Legs Along With Jaw Pain?

Question: Hello Doctor,
Iam 37 year old female.weighing 86KG and geight 5.5 inches My life is seriously stuck at a stage where I dont know who can cure me. I think Im going to be insane. In year 2010 April or may suddenly while doing my work at office I felt waves coming over to me starting from feet to head. I felt choking, racing heart and choking. I rushed to hospital in emergency room and they gave me some injection and a tablet and I became ok. But these episodes started very often and I went to neurologist. He prescribed Nexito (started from 5 and took it to 20). Petril SOS. I kept on eating that and for that treatment for about 7-8 years. Whenever I used to have anxiety i always felt nerve issue in my left hand. tingling in arms and left leg. Numbness in feet, sortness of breath, jaw pain and so on. Gradually I started being ok. But, then that doctor left and I went to another neurologist. He changed Nexito to Duloit 20.I took that for about 2 years. Then i developed cervical spondilitis and went for MRI. It showed some bulging in cervical disc and hence neurologist had put me on medicine Gabapentin NT, pregablin, fludac 20. He asked me to stop this after 2 moths which i did. But, since then I have acute attacks.
My BP is generally 110/70 but during that time i feel heat all over my face. Rush of blood under my eyes and my BP becomes 150/90. Pulse rate is on a higher side normally 100-105 but at the time of this attack it becomes 120-130.
This has happened to me thrice in past 20 days.
I dont know what iam upto.. Im very scared to live alone. I feel i will die. Yesterday also I had this attack and i had extreme chest pain with choking and pain on right arm. We went to clinic and he gave me a pinakiller injection and gave me Ecospirin 150. Has asked me to do stress Echo.
Iam hypothyroid since 15 years and taking 100 mcg dose. The other medications are glycomet 1000 twice for insulin resistence. My lipid profiel and sugars are normal.

Do you think I can ever be normal? what is it that iam suffering from? Is it some heart issue? Some hormonal issue? mental issue or what?

From the given description it seems that you are having panic attacks. Panic attacks are always due to underlying anxiety disorder.

You mentioned having sudden uneasiness 10 years back. Please tell me, if you can remember, what could have lead to such a panic attacks? Office stress? economic difficulties? physical problems? Fear of death or other illness related problems ?

You consulted many doctors and they gave you treatment for anxiety disorder and general symptomatic treatment. Please let me know the details of stress test.

In my view you need psycho education about anxiety and panic attacks, Cognitive behavioural therapy, relaxation therapy and lifestyle changes. I can provide you step by step instructions and tips over a period of 3 months.

You were in trouble for 10 long years. Please give me 6 months and I am sure you will learn to manage your problem.

Please provide othet information regarding your family, friends, job profile, general temperament etc

1- About me- Im eldest of 3 daughters my father has. We are only 3 daughters. My upbringing was very modern in terms of education, job and ambition. My parents wanted and expected a lot from me. I always had presseure to do best. I had to take maths and Bio both in my 12th standard so that I can give medical and engg exams both. But, I wasnt selected in any and did my BSC. followed by MBA. My father was a bank officer now retired and my mother teacher. We were happy family otherwise. Very much fun loving. I support my parents a lot fiancially, mentally and generally take care of all thier small needs.

2- When my panic attacks started 10 years back - The situations were bit strained between me and my husband as I had small 3-4 year old daughter at that time. Iam working and travel from XXXXXXX to XXXXXXX I had joint family as my in laws used to live with me. So we had bit of our issues with respect to responsibility of child, expecations, my job shifts and all. Above that I had a friend in my team who used to console me a lot. I was very close to him. But, people thought that relationship as different. My husband used to check my phone and all and somebody from my office informed him that iam going around with that guy. All in all professionally, personally situations were not good.

3- Now- we are very happy family, My husband loves me so much, my daughter is 11 year old and takes care of me and herself very nicely but yes injob there are unconciosus stresses always. People ask me that do I think about something, take stress or anything like that but I do not do anything like that deliberately. I get sudden attacks. Yes, I have noticed that I cant take physical stress so much which means if im physically very tired i get anxious.

4- My social circle- I just have very close bonding with my family whih is my in laws, my parents, my sisters and 2 pf my office friends. Apart from that I dont go anywhere in any party or anything but we go on family outings on weekends.

Please let me know your woking hours, leisure hours, at what time you leave to office and when you come back home, what is your job like, family life at home , how many hours you sleep a day, weekend routine etc

You must have to do all your household duties towards your children and husband and parents in addition to your professional tasks. Does this stress you a lot ? Do you experience insomnia? How is the sexual life ?

I will have to talk to you to give psycho-education. Usually I do it through Skype voice/ video call. Are you comfortable with that ? Please let me know.

My working hours- I leave home 6-45am in the morning and come back by 7-30pm. I have a domestic help so im not that much stressed from that part of life. It is manageable.

Sexual Life- Yes, its fine. We are in synch with each other and no troubles at all.

Sleep- Yes, I feel little difficulty in sleeping at night. Sometimes I have to watch something on XXXXXXX or chat with sisters so that i feel sleepy. I generally sleep 6 to 6-30 hours on working days but recover myself during weekends by sleeping in the afternoon or so.

Ya, video call can be fine with me but I can do it via whats app as I cant use Skype at office. Can you please tell me what medicines should I continue right now? and how will your sessions work if I plan to take them further?

I will tell you everything during our first video / audio session. Please keep in mind, we will have weekly sessions through telephone/ video. Every session could be for more than 30 minutes. First session will be approximately one hour. You need to make yourself available. Weekends will be better for you, it seems. We will decide timings as per both of our convenience later.

For the time being you should take all the medicines prescribed by other doctors. Please let me know the psychiatric medicines you take now for anxiety, depression or panic attacks.
