What Causes Enlarged Prostate?

Question: dear doctor.

I am a 36 year old male and a type 2 diabetic.

for last 4 days I have been going for a wee and feeling that I haven't emptied my bladder properly.

I have had a urine test and all full bloods taken and everything is perfect.

what could this be?

I am drinking plenty of water daily more than I am used to.

could this be why?

also a weird feeling on tip of penis after urinating.

I clearly understood your symptoms sir and would like to add a few others symptoms that may be you can relate to. Please write back to me informing me about which of them you have (for example: 1,2,5,8). The symptoms are:

1. Frequent or urgent need to urinate
2. inability to completely empty the bladder
3. Urinating at night more often
4. Having to have to strain while urinating
5. Certain amount of time taken to start urinating
6. Dribbling or poor stream of urination

Also, is your blood sugar under control sir? What were your last few levels and have you checked your HbA1c? I look forward to hearing from you in order to help you further.

I am not straining.

inability seems about right.

urine has a good flow.

there isn't a pressure pain. just feels weird at end of penis.

Then in my opinion, the possibilities include the following:

1. Enlarged prostate which normally puts pressure on the urethra, and prevents complete emptying of the bladder. This can be diagnosed with the help of an ultrasound and I would strongly recommend this investigation. Treatment includes the use of medications if the enlargement is mild or moderate, and surgical removal of the enlarged part if it is severe

2. Blood glucose irregularities: An elevation in blood glucose levels causes irregularities with the bladder and urination. I would recommend getting your blood glucose profile conducted to check if your diabetes is being well controlled or not (including a test called HbA1c)

3. Hernia: This is quite obviously diagnosed by a physical examination, but can be confirmed with an ultrasound of the abdomen and pelvis as well. I only recommend this because a hernia can cause a weird sensation around the tip of the penis.

Yes sir, you need to lower your HbA1c. High blood sugar levels can cause a number of complications, and this level could possibly be the cause for the bladder trouble you are facing. Although this needs confirmation.

Oh yes sir, prostate enlargement (although usually considered to be a condition seen in men around the age of 45-50 years) is nowadays been observed in younger men in their 30s

Not really serious, but depends on the cause sir. If it is due to the blood glucose levels, then consider this a warning sign from your body telling you that you need to make changes or else worse things are coming up.

If it is due to the prostate, it is nothing serious and can just be early onset of BPH (benign prostate hyperplasia).

If it is a hernia, then we can accordingly go ahead with surgical treatment and remove the cause completely.

Prostate cancer is a far shot and a more unlikely cause. Only if at all prostate enlargement is seen with the help of an ultrasound, then you will be tested for a test called PSA test. This is routinely conducted when any individual with prostate enlargement is found, to rule out prostate cancer.

sorry I am very anxious. even though these symptoms have only been for 3 days.

I am taking in plenty of fluids aswell. could this also be a cause?

By taking plenty of fluids like i have. Does this cause to go to the tooilet more?

I seem to go more when dribking 2 litres within 1 hour.

Is this right?

Yes of course sir, if you drink more water then you will visit the toilet more often.

I would recommend 3-4 litres per day sir, this is normal and healthy. But the normal amounts recommended are 2-3 litres, but I recommend at least 3 litres.
