Question: Hi. My wife (aged 37 years) has normal menstrual cycles, borderline DM Type 2 (taking metformin as prescribed), 12 kg overweight, happy living...
She sometimes has emotional bursts, moods ups and downs...recently she has started screaming on our baby (3 years) like never before, sometimes eats a lot, sometimes feel motivated so does diet n exercise both, sometimes cries without reason, feels guilt and low for nothing, says its something hormonal/chemical. As I am in Malaysia, has less medical facility here, cant take her for medical check up or lab investigations, kindly suggest

I think, she has bipolar disorder with current mixed episode. Let me explain about bipolar disorder. In bipolar disorder, patient can have two kind of mood episode either depressive or manic. In manic episode, patient has mood swing, easy irritability, increase appetite. decrease need for sleep, overtalkativeness, feels energetic etc. In depression, patient feels low mood, decrease appetite, decrease sleep, fatigue, guilt feeling etc.

Both kind of episode could be possible in bipolar disorder. When patient presents with both kind of episode at same time, then that episode is mixed type. Same may be the case in your wife. In that case, she needs mood stabilizer like lithium or valproate with low dose antipsychotic. These drugs are not available without prescription, so you have to consult a psychiatrist. Bipolar disorder is a mental disorder, so most of routine investigations would come normal. You can get her thyroid profile done.
