Question: My Anti Microsomal Antibody is 589.70 and my Anti Thyroglobulin Antibody is 37.40

My free t3 is 2.58
My free t4 is 0.81

plss recomend what shoulf i do

Please upload the reports. How is your TSH?

Your thyroid hormones ( free T3&4) are normal. Thyroid antibodies are elevated. The treatment here depends on TSH.

If TSH is above 5 miu/ml you need to be treated with thyroid hormones. If TSH is less than 5, no need of treatment. But you have to do thyroid function every 6 months, because you are at risk of hypothyroidism (under active thyroid) as your thyroid antibodies are high.

Hi XXXXXX,

Welcome back.

Your TSH is slightly elevated. This is called sub clinical hypothyroidism- TSH between 5 and 10 miu/ml and thyroid hormones hormones. One indication for starting treatment in this situation is increased antibodies in blood.

You can consult a physician and get started on a small dose thyroxine. In a similar case I will start on 25 mcg of thyroxine ( Eltroxin, Thyronorm, etc) - morning empty stomach.

Thankuu I would like to know how will the low dose of thyroxin help me.

Do I have to take it for all my life or is it possible that it will come down on its own with medicine

Hi XXXXXX,

I understand your concerns.

You are having Hashimoto's ( auto immune) thyroid disease. Here the auto antibodies are destroying your thyroid and decreasing its hormone production.

Now your thyroid is producing almost 90% of hormone needed for the body. So what we are trying to do is to REPLACE the remaining 10% by giving thyroid hormones. This won't improve your thyroid gland function, but will meet the thyroid hormone requirement of body to 100%.

We cannot stop the auto immune process, because it is already formed. All we can do is to replace the hormones as and when needed. More than 90% of Hashimoto's thyroid disease is progressive. That means your thyroid gland produce less and less hormones in due time and we have to increase the thyroxine dose. We cannot predict when your thyroid gland become completely under functioning. It may take months in some cases and many years in some.

All you need is to start on thyroxine and monitor your TSH every 3 months and increase or decrease the dose based on TSH.

So hypothyroidism is an easily treatable disease, but we cannot cure the disease process.
