Question: Good evening dr Hope you has my son history of adenoid. Now its 5th week he is in meta spray treatment. Yesterday evening he got ear pain and morning I noticed noticeble mild to mod swelling under his left ear.we wnet to pead dr he just used torch and told its not ear its some saliva gland infection and gave cepodoxime poxe pottasium calv orally dispersal tab 100mg 5.....half tablet as each dose twice for 5 days And combiflame for 5 days irrespective of pain. Is it due to adenoid he got ? Z the antibiotics is anf to control the infection fully or it is very less dose.Combiflame can I stop if he has no pain.or it necessary to give for 5 days and complete its course. As his adenoid not responded to antibiotic will this short period antibiotic will be enough to his infection now, or it become subacute it will be tough to come out. Presently he has no fever and normal active.

Brief Answer: Routine treatment as per symptoms. Detailed Answer: Hi, Thank you for your query. 1. Adenoid enlargement is seen children between the ages of 3 and 7 years. Now-a-days, with sustained medication including antibiotics, anti-inflammatory, anti-allergic, mucolytic, steam inhalation and steroid nasal sprays (such as metaspray), most of the cases with severe adenoid hypertrophy respond well. 2. As he has no fever and is active, it is not necessary to continue the antibiotic (after this short course) and the anti-inflammatory medication (Combiflam). The short course of Cefpodoxime and clavulanate will be sufficient to control any infection. 3. Get an X-ray Skull lateral view for adenoids done, If the adenoids are not enlarged, this is not related to the swelling under the ear. 4. The swelling under the ear may be due to a lymph node or a parotid salivary gland. A neck USG (Sonography) will settle the issue. It is important to rule out Mumps. It is better to get his ear examined by an otoscope (than with a torch). 5. These are very common childhood problems and respond well to treatment. I hope that I have answered your query. If you have any further questions, I will be available to answer them. Regards.
