What Causes Ear Discharge With Diminished Hearing?

Question: Dear Doctor, I am suffering from a white colour discharge and discomfort/heaviness and diminished hearing from my right ear since almost 1 .5 months now. First I showed to an ENT, who gave me otek drops for 5 days. I went to him after 5 days, but he said i will have to undergo an operation to remove the " massa" . Scared by the word operation, I went to another ENT for a second opinion on 7th March 2014. He kept me on tab zinox 200 and tobrex + miliflox DM ear drops for 10 days. after 10 days. the discharge continued after 10 days, hence he changed the ear drops to Vigamox. On 28th March, He asked me to do culture/ sensitivity as well as HRCT of right temporal bone. I can attache the HRCT report herewith. Please suggest me what is this disease and do we need to consider operation or not.

Brief Answer: Most probably a surgery will be required Detailed Answer: Hi, Thanks for the query. Ear discharge with diminished hearing with your ENT suggesting you a surgery points towards the diagnosis of Chronic suppurative otitis media which means that there is a perforation in your ear drum. Your CT scan report is also suggestive of the same and as there is a perforation in ear drum, it will be needing a surgery to close that perforation until and unless it is a very small central perforation. As your doctor has told you that you need to undergo surgery for a 'massa' chances are that there must be a polyp in your middle ear and that needs to be removed. Your ear is wet/discharging and not responding to treatment, it is rightly that your doctor has prescribed you a culture sensitivity which will help in specifically prescribing an antibiotic. So, please wait for the culture report and use that antibiotic. Afterwards if the ear becomes dry, the surgery will be required to close the perforation. If the ear does not become dry, then the surgery will be required to clean the mastoid bone as well as the middle ear and close the perforation. So, most likely you will be needing a surgery until and unless your ear becomes dry and the perforation gets healed on its own. Please keep your ear dry and avoid entry of water in the ear. Dear Mr. Duggal, Thanks for your detailed reply. My further queries post receiving your reply are : 1. You have asked me to keep my ear dry. but what about bathing everyday and still keeping the ear dry. as of now, my doctor did not ask me any precaution during bathing. 2. after removing a part of the massa and giving it for some pathological report, I have been taking the vigamox. surprising for last 2 days, my ear has stopped discharging and I have been feeling dry for last 2 days. 3. when the doctor had examined my ear internally, he had quoted that the ear drum seems to be good. But you have mentioned the existence of a perforation. How to conclude whether perforation is there or not. 4. If I go for the surgery ultimately, is it kind of a permanent solution or this disease is of recurring in nature. 5. Is it possible to re-gain the hearing ability lost so far by this operation or will I have to suffer progressive hearing loss even after the operation. Warm Regards XXXX

Brief Answer: Descriptive Detailed Answer: Thanks for the query. Placing a cotton plug while taking bath is the precaution which is recommended in discharging ears. Your ear has become dry after removal of massa points to two possibilities; one that the discharge was coming from massa which was occupying the ear canal and the ear drum was normal, second, the massa was coming from inside the middle ear through the ear drum and was not allowing the ear to ventilate and thus becoming dry. Removing it ventilated the area and helped in drying it. Which doctor told you that drum is good, first or second? Which doctor took a piece from massa? If the first doctor told you that ear drum is normal, it is not the possibility as massa must have been covering some part of drum. If second doctor took a piece, and told that ear drum is normal, that means massa was origination from external auditory canal. Then your CT should not show the evidence of mastoid infection. So, in my opinion, there must have been a small perforation which got healed when ear became dry which is many a times the case. There can be mastoid infection without perforation but the ear discharge need s to come from some where, so the possibility of perforation. Feel free to ask another query. Regards
