What Causes Dry Mouth And Dizziness When Diagnosed With Fatty Liver Disease?

Question: Dear Sir,

I am an XXXXXXX Male, presently staying and working in Abu Dhabi. I have given all the details as much as possible for your kind advice and further directions.

Age: 34 Years (16-9-1981)
Height: 166 cms (5 ft 6 in)
Weight: 94.8 Kgs (BMI 34)
BP: 145/95
Work & Residence Location: Abu Dhabi (UAE)
Work Condition: Almost Sedentary

Past Medical Conditions Description:

(1) Suffered from RENAL CALCULUS in 2003 due to OTC medicine used for Hydrocele

(2) Suffered from HEPATITIS (don’t know which one) in 2004 which was an epidemic happened in my hostel due to contaminated water and almost 5 people suffered at the same time.

(3) Diagnosed with FATTY LIVER – Grade 1 in 2013 due to heavy usage of Alcohol.

Present Medications:

(1) Concor Cor 2.5 mg (Once every Morning X 90 Days)

(2) Euthyrox 25 microgram (Once every Morning X 90 Days)

(3) Ursofalk (Ursodeoxycholic Acid) (Twice a Day X 60 Days)

(4) Movicol (One XXXXXXX at Bed Time X 20 Days)

(5) Cyanocobalamine (1000 mg) via injection every 2 weeks)

(6) Cyanocobalamine has been changed by me to Methylcobalamine (500 mg) tablets which I started taking 3 times a day (total 1500 mg) and now my neck nerve problem is far better relieved which I was not able to get after using one dosage of Cyanocobalamine injection. Its worth mentioning that this shift from Cyanocobalamine Injection to Methylcobalamine is not under the supervision of Doctor.

Present Medical Condition:

Very Dry Mouth and feeling dizzy after breakfast. Dry Mouth is resolved by Afternoon only. Mouth is so dry that i cannot eat without using water to push food down.

My Questions:

(1) What is the cause of Very Dry mouth? Kindly advice if any above medicines have interaction with each other.

(2) Can i take MILK THISTLE with URSOFALK or its not necessary?

(3) What quantity of alcohol can be used and in what frequency?

(4) Upon your expert review and evaluation of above said details and correlation with reports, what is your inference on long term chronic diseases of liver, ie, can you advice whether I am suffering from Fibrosis, Cirrhosis or any other liver disease.

(6) As I am doing diet control, so can I take Multivitamins (Ranbaxy Revital or Vitabiotics Cardioace) once daily or will it contradict with above said medicines?

(7) Last but not the least, If you feel that the above medicines are not necessary at the age of 34, then kindly advice your expert opinion on lifestyle changes or medications which may not harm on a long run.

Hi an thank you for asking on HCM!
I am Dr. Klerida Shehu gastroenterologist and I am glad to assist you.
I read carefully all your concerns and I can say as follows :
1. You are reporting to have had renal calculus which in recent abdominal ultrasound dose not exist. This is a good news.
2. Your current treatment includes ursofalk. I would like to know which is the reason that you your doctor has described it?
Because the indication of ursofalk use is gallbladder calculi or sludge . Neither of them is visible in your exams.
This is why I suggest to stop using it.
The best choice for fatty liver treatment especially a mild grade as in your case is Milk thistle plus healthy diet and lifestyle.
This is why I suggest to use milk thistle to help you liver.
3. With regards to Hypotyroidosis treatment you should continue to use euthyrox as described by your doctor.
4. You are reporting to use concor 2.5 mg. I would like to know the reason?
Are you suffering from Hypertension or tachycardia.
Otherwise I can say that concor has side effects like dry mouth. It can explain your symptoms if the dry mouth has started after concor use.
5.With regards to hepatitis that you have had in the past.It is hepatitis A and is only acute form.It is not associated with chronic liver damage as other hepatitis. So don't worry about it.
To my opinion you are not suffering neither to fibrosis or cirrhosis. You have only fatty liver that can be cured with diet and treatment.
6.You can use multivitamin There is no interactions with your current treatment.
Otherwise in my clinical practise I incurrruge my patients to use healthy diet and take vitamins and minerals from the food.
7. You are asking about alcohol use. I can say that at your case I would prefer to stop
it at least for 1 year. It is a need time to see improvement of fatty liver.
If you can't do this than you can use less than 2 drinks per week.
At the end I would like to recommend to use a healthy diet as follow:
- increase physical activiy
- try to loose weight since you are overweight
- use plenty of water
- use fresh vegetable
- avoid fast foods
- avoid sugar and fatty foods
- use home made dishes
- use salads and soup before consuming other kind of foods (especially for lunch)

Dear Doctor,

(1) Can Milk Thistle be used with Ursofalk together? Ursofalk was prescribed by my Gastroenterologist in Abu dhabi for my Fatty Liver.

(2) Concor was prescribed to me as my BP was 145/95 and pulse was 97 due to stress for some time. Kindly prescribe if any other alternative medicine is there to replace Concor.

Hi back,
Let me explain again :
1. You can use both ursofalk and milk thistle, there is no interactions between them..But as I said above I don't recommend you to use ursofalk for fatty liver. There is no evidence that ursofalk helps to improve fatty liver. This si why I recommend you to use only milk thistle.
2. With regards to concor use, since you have had high blood pressure than you can continue to use it.
Otherwise since your symptoms are related with stress it would be better if you start to use anxiolytic drugs like Xanax to help improving you anxiety.
