Brief Answer:
Your friend needs few investigations

Detailed Answer:
Hello dear, thanks for your question on HCM.
In my opinion mostly your friend is having GERD (Gastro Esophageal Reflux Disease) and bronchitis.
Both are due to smoking habit.
In GERD acid reflux occurs which causes symptoms like vomiting, stomach and digestion problems. It is due to laxity of gastroesophageal sphincter.
And bronchitis causes coughing and breathlessness.
So you should get done PFT (Pulmonary Function Test), chest x ray and upper GI endoscopy to rule out all these.
For this he needs to consult pulmonologist and gastroenterologist.
And smoking cessation should be the priority.
Hope I have solved your query.
I will be happy to help you further.
Wishing a good health to your friend.
Thanks.
