Question: I am 37 and male. In the last year I have been getting dizzy spells. At first only when I was playing sports or doing activities but lately it's been coming up at random. I also get an uncomfortable tingling in my stomach area at times. It not pain but it is uncomfortable. I also am very fatigued all the time

Hi! Welcome to health care magic! First of all, it is sad to know what you has been going through this lately.

Based on the history you have shared, this situation can fall into multiple reasons leading to such dizzy spells, however to start with you need to have an EKG to rule out any abnormalities with heart rate or rythm and that can lead to drop or fluctuations in blood pressure. History of chest pain also raises a need to rule out cardiac cause first. In later stages if suspicion of cardiac cause is high, holter monitoring may be needed in later stages.

The stomach discomfort on the other hand raises the need for blood workup for anemia or blood loss in the form of oozing gastric ulcer etc. which can lead to low hemoglobin levels leading to dizzy spells and easy fatiguability. Few other stomach conditions like gastritis can lead to low absorption of iron and vitamin B12 which can also cause low hemoglobin levels. To start with blood counts and workup for micronutrients deficiences like B12, D3 and folic acid along with thyroid levels may be needed.

The neurological causes may also be needed if any signs of vertigo or nystagmus or found on detailed examination by your doctor. A workup for benign positional vertigo may also be needed if such signs are found on detailed examination.
