Question: 36 year old female/Hashimoto's hypothyroidism since March 2001. Well controlled on synthroid. Normal pap test January 2014. Two healthy pregnancies/children... ages 14 and 10. In March 2014, I skipped a menstrual cycle. No symptoms, no discomfort, no PMS symptoms. I went to Dr. I was/am obese. At that time, I weighed 278 pounds. HPT negative. FSH run, but at unknown point in my cycle... in range as far she could tell. She believed weight was causing hormonal changes. Period returned--no more than 60 day cycle. I did diet/exercise/started phentermine, and I lost 40 pounds. Everything seemed to be going well. Now something else is happening. I have had sore breasts for 2 weeks. Nausea for 2 weeks. Heightened sense of smell for 2 weeks. Painless clenching (same areas that clench and release with orgasm)off et on for 1 week. Lower back/pelvic pressure for 2 days. Dizziness approx 2 weeks ago, but seems to have resolved after 2-3 days. Urinary frequency for 7-8 days. No cola colored urine. No urgency. No dysuria. Concentrated yellow urine. No foul odor. I do not know the first day of my last period. I generally get them at the beginning of the month. I do not generally have any symptoms other than sore/heavy breasts for 1 week and pelvic pressure 1-2 days prior to starting. Moderate flow for 2 days, then 3-4 days of spotting. Husband had vasectomy in 2006. He completed his specimen checks, and we were given the all clear. HPT negative. Hot flashes x 2 days. Left-sided sciatica yesterday only. Poochy belly/above waistline noticed 2-3 days ago-not normal for me. Pear-shaped body. No pain with intercourse. No issues with vaginal discharge. Clear. Still wet and slick, not watery like I generally observe 1-2 days prior to starting my period. Still obese at 237 pounds. No family history of PCOS. Menarche at14. 32 day cycles until on BCP at 17-20 y/o... then 28 as expected. When I was pregnant, I did not have any symptoms of PMS. I just stopped having periods. No discomfort at all or any symptoms I am experiencing right now. Again, vasectomyand negative HPT, although not sure when I should have started period, I doubt pregnancy as cause. Any thoughts? I feel like I am potentially having false-pregnancy? Perimenopausal at 36? I have been an RN for 12 years. I suspect a transvaginal sono would help... maybe CBC, CMP, LH, HGB A1C, FSH, HCG, INSULIN, TSH, T4, thyroid antibodies... even GTT...

Yes, all those tests are in order but some need not not be done and others can be omitted. So I will write a fresh list:

CBC
CMP
TSH
Free T4
anti TPO (Thyroid Per oxidase) antibodies
anti Thyroglobulin) antibodies
FSH
LH
Estradiol
Prolactin
HbA1c
Lipids

Based on results of above tests we may consider further evaluation for male hormones such as
Total Testosterone
Androstenedione
DHEAS

along with
17 hydroxy Progesterone
SHBG (Sex Hormone Binding Globulin)

You will be best served by seeing an Endocrinologist in-person who will have the opportunity to examine you thoroughly, and conduct a comprehensive assessment
