What Causes Dizziness, Headaches, Fainting Feeling, Weakness And Pinching Chest Pain?

Question: Hello Doctor , I am a 23 year old male , generally fit and healthy , I do not smoke or drink and I have had a normal full blood test two weeks ago .
I was diagnosed with heart rhythm problem three years ago ( PVCs that's what doctors call them , skipped beats ) and I have had a Catheter Ablation a year ago , after the ablation my symptoms have improved a lot and my heart rhythm is normal most of the time but I do get isolated exrta beats from here and there daily , cardiologists told not to worry about them as long as they are not frequent , my heart has been checked with echo and Cardiac MRI , stress test , and all came back normal .
My Current problem is unexplained dizziness , throbbing headaches , feeling of passing out , weakness , feeling irritated , pinching chest pains . This has been going on for a long time not and they don't find anything wrong in the emergancy room .
I am a very emotional person , easily get nervous and anxious , There are neurological problems in my family , my grandma had Parkinson's Disease , my aunt had Schizophrenia , this makes me worried as I believe I might be suffering from some type of Neurologican disorder and that is causing my daily symptoms . I have noticed that my heart rate speeds up when I swallow , also when I have this dizziness it gets so much worse when I stand up from sitting down or laying down, I feel immediate worsening of my dizziness and throbbing pain gets worse too , I feel my heart beat everywhere in my head and neck , i think my heart rate goes up to 100 - 110 when i stand up , but I only notice this during dizzy spell attacks , I can wake up feeling normal and then slowly start to get symptoms and by end of the day I have severe dizziness and feel Like I am going to faint , I was thinking about gettting a Tilt table test done ? Does this sound like Dysautonomia ???? Thank You

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I have gone through your detailed medical history.If you would have presented to me as an outpatient,I would have told you that some of your symptoms like irritation and pinching chest pains are probably resulting from anxiety.Please start doing regular mental relaxation exercises like meditation/yoga and it should bring at least partial improvement in your symptoms.

Further,your dizziness specifically relates to the posture as it gets precipitated in standing position.Autonomic dysfunction should be ruled out,as these symptoms are specific for autonomic involvement.Please get yourself evaluated for postural fall in blood pressure.Tilt table test may be helpful for ruling in or ruling out the diagnosis.
Besides,please tell whether your throbbing headache is also associated with dizziness?Do the headache and dizziness occur at the same time?If not,please describe the headache in detail-What is the frequency of headache?What are the precipitant factors?Is the headache unilateral or bilateral?Is the headache associated with nausea or vomiting?

Usually it starts with dizziness and then headache follows , and yes they are present at the same time. It will subside a bit when I lay down and get worse when I get up . There is no nausea or Vomiting feeling , throbbing speeds up when I stand up and that's when it hurts the most , it almost always starts in the evening time and will last hours , after getting sleep at night I do not feel rested , it feels like If I have not gotten any sleep . This morning my heart rate slowed down with standing up from laying down after 15 minutes , it got slow and forceful , heavy beats for couple of seconds until it returned to normal , I have had this happened three or four times before , one time at night I got out of bed really fast and immediately I felt my heart slowing down and beating exteremly heavy ..... Can you PLEASE help me out Doctor as I have gone to the emergency room many times and they never ever make any diagnosis .

I have gone through your detailed clinical history and I am of the opinion that you should undergo autonomic functions evaluation including-
1)Postural drop in blood pressure
2)Tilt table test

Further,
Throbbing headache lasting for hours is not a feature of autonomic involvement.Considering this,a trial of anti-migraine therapy should be started to observe whether it improve the symptoms or not.

Hi Again Doctor , Before I close the thread I'd like to ask you a few more questions please .
I have forgotten to tell you that often my dizziness is triggered by looking up , if I am shaving my face and I have to turn my head up or on sides there I get immediate dizzy spells , what could cause that? I will try to get a referral for Tilt table test but I did this test at home , I was laying flat for 30 Minutes monitoring my heart rate and had blood pressure cuff on my arm , while laying down after 30 minutes my blood pressure was 109/58 pulse 59 after standing up immediate readings were 135/74 pulse 98 , after few seconds pulse went down to 60 . Is that normal ???

Head/neck movements triggering dizziness goes in favor of positional vertigo,but in typical 'positional vertigo',the episode of vertigo is of very short duration(lasting for seconds) and very intense in severity.So,this has to be observed for some time.Anxiety may precipitate such events.So,try mental relaxation exercises and see if it still persists.

Tilt table test is not consider positive in absence of syncope.The cut-off points and sensitivities vary according to different protocols.So,it has to be done under a neurophysiologist with well established protocol,to say it positive or negative.

GP has referred me for the Tilt Table Test but it is going to take two or three months to get an appointment , meanwhile she said I can try Stemetil 5mg or Sec 16 Mg three times a week , I looked up side effects of these medications that treat vertigo dizziness and they have cardiac side effects and that worries me , can you please give me a name of a good vertigo dizziness medication I can try before I go fot tilt table test , this is my final question thank You .

You can take either Cinnarizine or betahistine on sos basis(at the time of symptoms).PLEASE TAKE ALL MEDICINES UNDER A PHYSICIAN'S GUIDANCE ONLY.

Along with,please take plenty of fluids as that helps improving in some of the autonomic symptoms.
