What Causes Dizziness, Headache And Blurred Vision After Taking Telma?

Question: This is a follow up Question of Transaction I'D no-HCM 736444.
My wife is taking presently Telma 40 mg after settling of BP with Telma AM.
But she is feeling Dizziness, Headche & blurred vision.I have so many times told to cardiologist about this but he is reluctant to change medicine.
May please advise as my wife is always complaining about this.

I apologize for the delayed response owing to my work schedule.

These symptoms are directly related to the side effects of the drug.

Many people show features of lowered blood pressure or hypotension such as dizziness or headache.

At this point, it is important to lower the dose of TELMA to 20 mg and observe for symptoms over a period of 5 to 7 days.

Many symptoms resolve with dose reduction.

In case if they still persist, its important to change the drug once for all.

Please check with your physician if he shares my view and if can make these changes to her.

Dear sir,
Thanks for response.
My wife BP is raising & after some time it comes to 130/90.Actually we are measuring BP with dizital BP machine so it may have device error.
would it be advisable to go to Telma 20 in this situation?
We are concerned about lowering of medicine but it is also true that Dizziness,blurred vision & headce is present.
Can u advise some medicine which will have less side effect & will also work so that in next follow up I can discuss with doctor.
Recently glass power is also changed when cardiologist advised to see ophthalmologist ,as pt is complaining about headce.

Please resolve the illusion.

I can certainly understand your concerns.

I apologize for the delayed response owing to my work schedule.

I worked through your query in detail.

First and foremost, do not rely on Ayurvedic medicine. It is incapable of controlling the Blood pressure.

At this point of time she can get on with TELMA 20mg.It is advisable to keep the BP in the range of 110-130/60-90 mm of Hg.

High blood pressure can certainly lead to pregnancy issues if only not properly managed in time.

Once she is a pregnant, TELMA has to be changed to other suitable antihypertensive as TELMA should not be considered during pregnancy as it can affect the growing baby.

The rash on the ankle is related to drug TELMA.But this rash will subside on its own.

I would like to know if she is taking any other drugs beside TELMA?

Further, I would like you to send me our previous complete conversation in the form of a screenshot on the upload report column as I am unable to retrieve it on my smart phone.

I request you couple, not to worry about the next pregnancy as excellent treatment options are available.

Yes, even eye sight issues can also lead to headache and occasional dizziness.

As of now she requires a complete FUNDUS EXAMINATION of the eye through the indirect opthalmoscopy procedure. Get her this test and update me with the results

TELMA is an excellent drug for her at the moment. I advise your wife to take 20 mg for 2 weeks with monitoring of her Blood pressure every alternative day and make a note down on it and intimate me in your follow up.

Tell your wife not to become stressed as things can be brought under control to a great extent.

Weight reduction to 60 kgs can help to control BP to great extent.

This can achieved after 2 to 3 month period.

I advise her to have a brisk walk daily for 20 min for at least 5 days a week.

Oatmeal for breakfast on a daily basis will help to control her BP and weight to a great extent .

We will think of changing the medication only after 2 week observation.

As of now continue TELMA.

The rash can be controlled with oral pills of antihistamine drugs like LEVOCETRIZINE.for 1 week.

Direct telephonic conversation is allowed only for the internal doctors of HCM.

I will guide you through do not worry.

Please check with your physician if he shares my view and if can prescribe the same to you.

Further management if any is largely based on test results that is Opthalmoscopy.

Hello.
My wife age 31 yrs ,Height 5.2 inch ,weight 72 kg .She is suffering from hypertension .Due to disagreement in opinion between us she became anxious.On 30.09.2015 her BP was 160/90 & is prescribed PRAZOPRESS XL 2.5 & Telma-AM (Telmisartan 40 mg, Amlodipine besylate 5 mg) .She is also taking Thyronorm-12.5 mcg.The doctor prescribed both hypertension medicine saying that BP will not come in control but gradually I will wean off the medicine.she is also taking Irozorb plus.o
On 30.10.2015 her BP was 130/82.
Report of test carrird on 30.10.2015 are as follows:-
Haemoglobin-12 & Sugar fasting -88.T3 = 107,T4=9,TSH=4.19 .Her Thyroid test was ok but doctor suggested to take for 1 month & then stop it.
From Last 1 week she is complaining that headache & dizziness with problem in eye.We consulted Eye specialist & prescribed -1 in R & L both but she is complaining headche & dizziness (Chakkar)
Eye doctor suggested that headche may be due to setting of glass but i could not understand about dizziness may be due to raising of BP but her BP is now normal 120/80.
Seeing the situation I consulted another doctor who told that BP has fallen down due to intake of these two hypertension medicine,so stop prazopress XL 2.5 & continue Telma-AM.Due to dizziness & frequent urination we stopped Thyronorm & BP medicine for 2 days before meeting new doctor.The new doctor approached on 12.11.2015 that there is no problem of high bp because bp has not rise drastically even sudden stoppage of medicine.
So at present take Telma-AM & after 7 days I will see bp & then I will judge.
So from 13.11..2015 she is taking Telma AM but she is feeling dizziness. May please advise & set action plan for me.Should I change doctor ,I am in dilemma. I am living in XXXXXXX a small town so I may consult some doctor at XXXXXXX if u will advise.

I have gone through your query in detail.

This isn't a major issue unless properly managed.

First and foremost, stopping the intake of PRAZOPRESS XL seems to be a good move.

The dizziness could possibly be related to the effect first dose hypotension secondary to the 3 anti hypertension medications.

It is advisable to go with the opinion of your latest doctor. Watch for 1 week while on TELMA AM to decide the further course of action.

It is not advisable to stop thyroid medications abruptly. This can cause rebound elevation of blood pressure.

It is advisable to continue thyronorm 12.5mcg .

Now today ie on 05.01.2016 we have visited opthamologist & on checking minor power change is found & some cylindrical power is added.
So eye sight problem is checked & proper glass will be made.Today pt is also complaining about urticaria with tighten & swollen skin in ankle,face & chest.
I consulted dermatologist he ruled out possibility of Allergion due to Telmisartan .Wife is not taking any medicine except telma 40.She was feeling itching & given IM of avil+dexona.
Some ayurveda medicine is also taking like Ashwagandha.
Now I am going to proceed for telma 20 as she is complaining of dizziness but doctor is saying telma is safest medicine for BP.
As advised I will keep record of her BP & then will decide.
Now urticaria is new problem for her.But I hope with your guidance I can overcome problem.
At present my age is 34 yrs & wife is of 31 yrs with 1 male child of 4 yrs.
In 2017 we are planning for next issue.
I can request to HCM desktop for call arrange if u permit.
Any time suitable to u that will really give emotional support.

Amlodepine is known to cause urticaria and ankle swelling.

As earlier suggested, take oral pills of LEVOCETRIZINE for the control of Urticaria.

Continue low dose of Thyronorm @12.5mg.Repeat Thyroid hormone profile.

I have no issues speaking to you over the phone if you can convince management of HCM and arrange for the same.

Make sure her BP and thyroid hormones are well within the normal limits before she conceive in 2017.

Update me with her BP values and fresh thyroid profile.

Dear sir,
As Amlodepine is known to cause urticaria and ankle swelling. So please advise other safe high bp medicine that will certainly be discussed with my doctor.Continue low dose of Thyronorm @12.5mg.Repeat Thyroid hormone profile.
Her 15 days back TSH repeat test was normal so does it hold good to take thyronorm 12.5 mg.
Wife is also feeling irritating as she is suffering from hypertension in young age.
Whether I should continue ayurveda or not ?
She is also irritating due to intake of so many medicine.

Let's not substitute Amlodepine at this point.

It's advisable to go with TELMA alone at this point.

Additional medication can be considered only after verifying the BP values over 2 weeks.

Discontinue Ayurvedic medication as it does more harm than good, since many ingredients can worsen urticaria.

In case if she had TSH checked 15 days ago, it is important to get it checked after 15 days. Monthly Thyroid profile monitoring is more scientific to decide dose of thyronorm.Till then continue @12.5mg.

It's important not to discontinue Thyronorm when primary hypothyroidism is considered as a diagnosis.

I appeal to your wife to stay calm and adhere to medications. I can certainly understand her irritation towards polypharmacy.

Her BP has to be carefully dealt by periodical evaluation of the entire cardiovascular system and the thyroid hormone status.
