Brief Answer:
middle ear disease due to sinusitis.

Detailed Answer:
Hi, thanks for your query.
In my opinion, you might be having chronic sinusitis , causing eustachian tube dysfunction causing negative pressure in the middle ear.
This cause tinnitus, ear pain, and reduced hearing and ear block or pressure.
Once the sinusitis is treated, other symptoms gradually reduced.
I would suggest you to consult with the ent specialist and get yourself throughly evaluated.
If I were your treating doctor, I would have first clinically examined you, confirmed my diagnosis, then I would suggest you to get blood work, CT SCAN of sinuses, audiometry and tympanometry and Eustachian tube function test.
Depending on the report, I would treat you with antibiotics +decongestant tablet + decongestant nasal drops + antihistamines + steroids nasal spray. If symptoms persists, then I would recommend surgery to clear the sinuses.
Mean while same treatment holds good for ear, if it doesn't improve then myringotomy and ventilation tube insertion is recommended.
Hope my answers have helped you. Pleased to help you further. Thanks.

Brief Answer:
Need inner ear evaluation.

Detailed Answer:
Hi, thanks for your follow up query.
If middle ear is clear, then inner ear disease also can produce symptoms of dizziness and tinnitus.
This needs clinical evaluation.
I would suggest you to consult with the doctor and get evaluated properly.
Without clinical examination, it's difficult to diagnose the disease.
Emperically you can take betahistine + labyrinthine sedatives like cinnarizine 25 to 50 mg bd for 5 to 7 days, which will help you symptomatically.
Hope my answers have helped you. Thanks.
