What Causes Dizziness Along With Oscillating Sounds In Ear?

Question: After riding in my car for , say over a half hour, when I get out of the car, frequently I will have a minute or so of pre-syncope, also accompanied by the volume of my hearing oscillating significantly. (I have never passed out, but it's significant light-headedness. And I can exercise hard and be fine. Just passed a stress test with my heart rate at 134.) If a truck is driving by, it sounds like someone is turning a volume dial between full volume and 50% or so. Possibly this is in sync with my heart beat. I also have and have had for quite a while, tinnitus, although it is worse in the last couple of years. It is pulse synchronous tinnitus. I have seen physicians (GP, cardiologist, and ENT), had a brain MRI through the ENT. Didn't see anything. So I occasionally search online to see if I can find any clues. Sometimes it's just a matter of finding someone who has figured this out before, while most can't and haven't seen it before. This has been going on about 2.5 years. In between,(7 mo ago) I had a stemi which worked out fine, two stents, no visible damage. I'm in really good shape. Any ideas?

Looking at your history this could be meneire's disease or inner ear disease due to inadequate drainage of endolymph in inner ear

I would like to rule out other possibilities of this condition beforehand like high blood pressure, diabetes or generalised anxiety disorder

In my opinion after ruling out above mentioned conditions you should visit a vertigo clinic for following tests like
1.rotatory chair testing
2. Videonystagmography
3. VEMP vestibular evoked myogenic potential

Various treatment modalities are available for its definitive cure like
Motion sickness medications
meneiett device implantation
surgery like endolymphatic sac procedure
