Question: Hi i am 28 years old female and i have dark area around the mouth. This discoloration looks very odd. I have been visiting a dermatologist since last 2 years but the moment i stop the medicines and treatment the discoloration comes. My doctor had put me on on external application creams like clearz plus,Ap gel, melalite,amylac,tarzet(2min-7min),sepfil,epishine serum, white objective serum, melaglow, kozimax, strea c10,momisoft as well as laser treatment and peels(c peels, sds+kh, F+F.gm,G+Mns etc) in the last 2 yeard. However i stopped from last 3 months and the discoloration has come back. I have spent a lot of money on removing this but without any permanent effect. Would like you to tell me how i can get rid of permanently.

Brief Answer:
I suggest Retinol peel along with kojic acid.

Detailed Answer:
Hello. Thank you for writing to us at healthcaremagic

Perioral darkness is frequently constitutional in a lot of individuals just like periorbital darkness.
Pigment lightening agents may be effective but sometimes only for as long as one uses these agents, therefore, maintenance treatment may be required in most either with pigment lightening creams or with maintenance chemical peeling sessions.
I have done retinol 4% peel in my patients with good results i.e Retises-CT peel also commonly known as "yellow peel". You may inquire about it from your treating doctor. A session once every month is sufficient.
Topically you may continue with kojic acid preparation along with yellow peel sessions.
Once significant improvement has been achieved with peeling sessions you must still continue with kojic acid preparation for maintenance of improvement.

Hi
Thanks for the reply.

Is their a treatment which can be done at home?

Can this peel be done by myself or i have to visit doctor? Because when we visit doctor they come with alternate opinions and suggest multitude of remedies.

Brief Answer:
I suggest retinol 4% peel along with skin lightening creams e.g kojic acid

Detailed Answer:
Hi.

That is a very relevant question.
Doctors may advice an alternate solution based on their experience on how they have treated other similar patients in past.
Therefore, it is important to align your expectations with what your doctor has told you so as to avoid future disappointments.
Moreover different patients respond differently to treatment. A particular peel may work well in one patient and not so well in another patient.
Therefore, there is always an element of trial in each peel.
In general a peel ingredient like glycolic acid, mandelic acid, retinol, kojic acid, azaleic acid are all pigment lightening agents.

I can see that you have undergone sds peel (which is a salicylic peel; it is for acne and not for pigmentation); G+Mns (probably it is Glycolic acid + Mandelic acid+salicylic acid combination peel; it is quite good for pigmentation).

I am not aware of F+F.gm?could you elaborate on the peel ingredients. Also let me know what is kh in sds+kh?

Retinol peel (Retises-CT peel; It is marketed by Mediderma in India) contains 4% retinol. It is a vitamin A derivative.
It is a skin lightening peel and quite effective for both perioral darkness as well as periorbital darkness. You have to take it from your doctor.
After going through the treatments tried I can see that you have used a product from Dr. Reddy's lab which contains Vitamin C in capsule form which you have to break open and apply once daily. There is another product by the same company which contains Vitamin A in capsule form; you can try this. You need to apply it once daily at night after breaking open the capsule. This is same as retinol in yellow peel, which I had suggested earlier. You may talk to your doctor regarding a prescription of this.
Of course topical lightening creams e.g kojic acid based cream Or azaleic acid based cream, need to be used together with peels and for maintenance of improvement.

Can you upload a picture of the affected area? so that I am able to understand the severity of your problem and therefore guide you better
