What Causes Difficulty In Exercising Post Stent Replacement?

Question: I have not started my new medication, which zetia and raneza, but will start soon. My complaint is always intolerance of exercise after my stent. btw, have you received my two reports yet? I am a little hesitate to take the new medication because of the side effects of Lipitor that I had been taken. thanks again for your help!

I have received your reports.
So, your main complaint is intolerance of physical activity, breathlessness, and the main clinical findings are: moderate mitral regurgitation, mild diastolic dysfunction, dyslipidemia and NO ischemia.

If you were my patient I would consider two possible causes of your complaints;
1. Mitral regurgitation
2. diastolic dysfunction

For mitral regurgitation I would suggest cardiac surgeon consultation.
For diastolic dysfunction I would add ACE inhibitor.

Your doctor prescribed you Zetia for control your cholesterol level as you experienced side effects from Lipitor. I would advise to try it, if there is a side effect, then you should withdraw it. But there is a possibility you will tolerate it well.
I think your doctor prescribed Raneza, as he suspected ischemia. Anyway Raneza can improve slightly breathlessness caused by mitral regurgitation as well.

Thank you for your detail and prompt reply. I did not understand why my doctor prescribed Raneza if my test did not show ischemia. and why he did not prescribe me ACE inhibitor. Can I still exercise given of my condition? I know exercise is good for my heart, but I am really tired after the exercise. I have also upload one of report from the end of last year. Can you please see if there is any great different from my recent report. My doctor said eventually I need to have a surgery done. Is it because of that you suggest me to look for a heart surgeon. What can he do for me? What should I be careful about, given of my condition. Is there any different between cardiologist and heart surgeon? Thank you again for your valuable information. You make it so easy for me to discuss my case with my doctor.

Dear XXXXX,

There are no significant changes on your recent echocardiography comparing to your previews examination.

I don't know exactly, but I suppose that your cardiologist suspected ischemia as you couldn't reach your maximal exercise capacity. Anyway I would send to angiography in case of such suspicions.

I can't say anything about ACE inhibitors. Maybe he was afraid of hypotension, in case of prescription of ACEI together with Ranolazin (Raneza).

You should discuss with cardiac surgeon the possibility of mitral valve repair.
If surgery is not done or before surgery you should do prophylactics for bacterial endocarditis. You can discuss it with your doctor.

Thank you for your reply. I went to see my GP today and I brought about my concern about Raneza and he agreed that raneza will not do anything for me since I am already on blood pressure medication, eg. carvedilol. He also asssured me that i should not worry too much about anything right now since my report are very good and I am worried too much. He sees patients with that kinds of problems all the time. I am confused. I might want to seek an second opinion. Thanks, for the answers. I did learn a lots about heart problems from you. You think I should stop doing exercises? Thanks again!

Exercise is good for you. Choose an average exercise which doesn't make you breathlessness.
I think you should take your medications for a month. If you don't get better in a month, refer to your physician to discuss the possibility of changing Raneza to ACEI and meet cardiac surgeon.

ok. thanks for valuable advice. I will definately take up your advice. I will take it and see if they make me feel better. I was warned about the side effect of the raneza which is dizziness. My blood pressure is usually 110/70

I am always warned not to take too much of Vitamin K, because of the medication, eg. Clopidorel 75 mg and baby espirin 81 mg. So, how much is too much and what is considered a reasonable amounts? I bought some dry XXXXXXX powder, is 1 tsp of that powder is ok? I have eaten very little vegetable because of my fear of too much Vit K. Thank you for your reply, you are so great!

It is OK to take one tsp of XXXXXXX powder.
You should also know that there is no restriction of Vit K intake for the patients, who take Cloprdogrel and Aspirin.
Vit k is bad only with Vit K antagonists, such as Coumadin.
Aspirin and Clopidogrel have other mechanism of action.
