Question: Hi Doctor,
I am getting 10 to 15 sneezes immediately after wakeup. Also unable to breath properly with nose and taking breath from mouth. Recently I consulted ENT doctor. After my X-ray, he told I have cold allergy(cynus) and some puss formed sides of nose. He gave below medicines for 5 days and suggested me to have scan after 5 days to know whether this can be cured by medicine or by surgery.
1. Cefpodoxime Proxetil and Potassium Clavulanate tablets(2 per day)
2. Paracetamol, Phenyleprine & Levocetrizine tablets (2 per day)
3. Xylometazoline Hydrochloride Nasal Drops I.P ( 2 drops : 3 times per day)
After taking above medicines, I am still having same problem. I am able to breath with nose only after having nasal drops 3 times per day. Could you please suggest me whether this can be cured only with medicine or need of surgery.

Thanks,
xxxxxxxxx

Brief Answer:
Surgery is required if there is any deviation of nasal septum or polyp

Detailed Answer:
Hi XXXXXXX
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. The medicines prescribed to you includes an antibiotic, medicine for pain and allergy and drops to control allergy in the nose.

2. In the x ray we only see some of the areas and make a probable diagnosis. It is probable that there is increased density over the paranasal air sinuses showing that infection is there and that is the reason your doctor is giving you treatment.

3. A CT scan of the paranasal air sinuses is important to know the details of the structure of nose and sinuses. This can be done when you get better and will show any cause of nose block and difficulty in breathing other than the sinus infection. If there is any cause like significant deviation of nasal septum, bnlockage or polyp in the paranasal sinuses on CT scan then surgery is required otherwise it can be treated with medicine.

Hope your query is answered.
Please do write back if you have any doubts.

Brief Answer:
CT scan PNS will cost 2000 to 3000 INR

Detailed Answer:
Hi XXXXXXX
Thanks for writing back with an update.

1. You have to get a CT scan PNS (paranasal sinuses) Plain in coronal sections done. This costs about 2000 to 3000 INR in XXXXXXX

2. Surgery is suggested if there is problem on CT scan after discussing with the ENT doctor and is usually for any treatable cause of nose blockage like deviation of nasal septum or sinonasal polyps.

3. Sneezes in the morning and itching in eyes is likely to be from allergy and can be treated. Please get an absolute eosinophil count done to know if you have eosinophilia related to allergy.

Hope your query is answered.
Please do write back if you have any doubts.
