I read your question and i understand your concern. Considering the symptoms provided, bowel movements difficulty with clear discharge is likely a bowel disorder. If there is no history or complaint of incontinence then its likely a functional bowel diseases like IBS.

It needs some work up for ruling out etiologies like IBS or any incontinence. Pain from hip to hip or difficulty in lower abdominal muscles use may indicate excessive straining during defecation or they may be a separate entity. Take some muscle relaxants and acetaminophen for that and see if it helps.

Meanwhile, use fibrous diet, stool softeners, plenty of fluids and cranberry juices should be continued too. Avoid fatty food or food rich in dairy. See if it helps. If not then you will need some work up and a complete clinical correlation by an expert gastroenterologist for further guidance.
