Brief Answer:
Take another pregnancy test, urine test to check for infection

Detailed Answer:
Hello,
Thank you for your query.

I'm Dr. Shuba Hariprasad and will be answering your query today.

Please take another pregnancy test right away. If negative, please take a urine test to rule out urinary tract infection..

If your period doesn't come, repeat another pregnancy test when you have been a week late. If negative, see your OBGYN to rule out ovarian cysts and hormonal imbalances which can delay periods.

Stress, illness, travel, sudden weight changes, travel, major lifestyle changes can all delay periods.

Hope you test positive for pregnancy.

Drink plenty of water, avoid irritants like caffeine, sugary, carbonated drinks and spicy food.

Hope this helps.
Please let me know if I can help you with anything else.
Best wishes in your journey trying to conceive.

Wish you good health.

Brief Answer:
Period is on its way.

Detailed Answer:
Hello,
Thank you for the follow up.

Sorry, but it looks like your period is on its way.
You should have fresh bleeding within the next 24 hrs.
How are your urinary symptoms? Did you have an urine analysis done?

For the next cycle, try using basal body temperature monitoring & ovulation prediction kits to pinpoint your ovulation date.
Time intercourse on the day of ovulation and 3days before it (at least once in 2 days) to improve your chances of conceiving.

If you have been trying to get pregnant for more than 6 months, please consult an OBGYN to make sure everything is alright.
An ultrasound scan- follicular study with relevant hormone levels on day 2/3 of your cycle can show the status of your ovarian reserve, you follicle development and ovulation.

Hope this helps.
I'd be happy to help you with any further questions you may have.
Wish you good health.

Answered by Dr. Shuba hariprasad (3 hours later)
Brief Answer:
It will happen

Detailed Answer:
Hello,

I completely understand your disappointment. I realize how important this is for you.

Please take it one day at a time (easier said than done). The two week wait after ovulation can seem like eternity. This is when you have to keep occupied.
There are forums available online in which women discuss their symptoms, feelings, anxiety, hope, disappointment and keep each other company during this period. You may want to have a look.

Don't lose hope. Keep trying. Listen to your body. Eat healthy. Exercise in moderation. Take folic acid supplements. It will happen.
Please let me know if there is anything else I can help you with.

Wish you the best.

Brief Answer:
Please start having intercourse.

Detailed Answer:
Hello,
Thank you for the follow up.

Your follicular study indicates that you have multiple follicles developing. Usually by day 10 of estrogen follicle emerges as the dominant one which goes on to become mature and ovulate.

Did you check any hormone levels on day 2or 3 of your cycle?

I recommend that you complete the follicular study confirming ovulation and get back to me with the complete report. I can advice you better then.

I suggest that you have intercourse from now till your ovulation has been confirmed (once every alternate day at least).

Please also start to monitor basal body temperature and charting it to better understand your cycles.
Start taking Folic acid supplements as well (Tab. Folvite 5mg once a day).

Will guide you based on the results of the follicular study.

Brief Answer:
Please refer below

Detailed Answer:
Hello XXXXX,

Basal body temperature monitoring :
- you will need a digital thermometer with one decimal point in the measurement scale. Get a good one.
-check you temperature orally (under the tongue on either left or right side) as soon as you wake up (before you get out of bed, drink water etc). If you snore or tend to keep your mouth open during sleep then check your temperature vaginally.
-mark these values on a chart and it will give you a graph of your temperature fluctuations. Fertility friend is an online site which helps you do this online for free.
-once you have done this for a couple of months, you can observe your ovulation date and luteal phase length and fertile period.

In addition to this, also check for any vaginal discharge. Usually, during the fertile period and ovulation, discharge become watery or like egg white and resembles semen.. This helps sperm to pass easily into the uterus.

If you don't conceive this cycle (hope you do) , on day 2 or day 3 of your next cycle, check LH (leuteinizing hormone), FSH (follicle stimulating hormone), E2 (estradiol) & AMH (anti mullerian hormone).
Please note that day 1 is the day when there is bright red full flow of blood, not spotting.

-did you conceive your first child normally?
-do you have any medical conditions? Any other medications?
-any family history of diseases or infertility?
-what is your height and weight?
-any previous history of diabetes during pregnancy or other pregnancy complications? Was it a normal delivery? Is your child 6 yrs old now? Did you use any contraception during the past 6 years?

Please get back to me with any further questions you may have and the above requested clarifications.

Best wishes,
Dr. Shuba Hariprasad

Brief Answer:
Polycystic ovaries is a possibility

Detailed Answer:
Hello XXXXX,
Thank for the clarifications.

Your weight is fine. Have you checked blood glucose levels recently? Were they normal?
If not, please check
-HbA1C
-Fasting blood sugar
-post Prandial (2 hours after food)
-TSH, free T3, free T4 (thyroid function tests)

If anything abnormal we should do glucose tolerance test.

Multiple follicle development could mean Polycystic ovaries or hormone imbalance. A day 2 scan and hormone test will give better idea as will a follicular study. Based on that I can advise you further.

Please keep having intercourse till scan (follicular study) shows signs of ovulation. The mature follicle should be around 18mm to 30mm.
After this follicular study is over, please attach complete report.
Also mention your last 6 period dates..

Good luck,
Regards,
Dr. Shuba Hariprasad

Detailed Answer:
Hello,
Sure, I am available on weekends in the evening between 4 & 5 pm. Please call me (sms if I don't answer) on Friday evening. Will give you an appointment.
Number is 0000

About your follicular study, One follicle has ruptured but size was smaller than expected. You have a good sized follicle developing (F1).
Endometrium is triple lined (good for implantation).
Please continue intercourse and continue follicular study to see if the first follicle ruptures.

Do bring all your reports when you visit me.

Brief Answer:
Continue Intercourse and follicular study.

Detailed Answer:
Hello XXXXX,
A very good day to you too.

I've noted the follicular study findings.
Fluid in the pouch of XXXXXXX (pod) in addition to collapsed follicle indicates ovulation. However, the one remaining follicle seems to be still growing.

I think this will be a healthier egg but may not rupture due to hormones released from ruptured follicles.
Please continue intercourse and follicular study till this has ruptured as well or stops growing /reduces in size. (at least next 3 days)

Hang in there...
Take care.
Regards,
Dr. Shuba Hariprasad
