What Causes Delay In Language Development In Early Childhood?

Question: Dear Doctor,

I am working woman. I have a son of 21 months old. He is very active and healthy. I have maid to take care of him during day time.She seems to be ok.

His physical growth seems to be fine.
1. He is bit taller to his age. People ask me if he is 2-2.5 years old.
2. His weight is 11 kgs.
3. He has got 4 teeth in the top and bottom. And he is getting few more teeth now.
4. He walks and runs freely.
5. He can climb and get down chair/cot/steps.

His current activities are
1. He likes play football and other games alone as well in the company of parents and maid.
He has special liking towards vehicles specifically car.
2. He can identify almost all the part of body on him and on others.
3. He recognizes people when I show him pictures.
5. He responds when we call him or understands if we instruct him to do small jobs.
4. He can identify all familiar vegetables, fruits, birds etc.
5. He knows when he gets pee.
6. He knows eatable and non-eatables. He never tries to bite something unwanted stuff.
7. He indicates if he is hungry or thirsty by putting finger in his mouth.
8. He operates mobile/remote/radio. Plays song for himself. Makes phone call to grandparents.
9. He does handshakes, hugs parents etc.
10. When he sees strangers or not frequently seeing relatives he takes time. But he gets along with them.

Overall I don’t have any issue about all these things.

Now coming to my concerns.
1. He seems to be very sensitive. If I raise my voice for not his mistake he cries a lot. Does all kids are like this?
2. Sometimes he gets completely lost in rhymes on TV or computer, those times he does not pay attention at all if we call him. Is this normal?
3. He likes circling himself alot. He does it so much, I get scared thinking he might fall down due to giddiness. Is this ok?
4. He speaks local language words like appa-father, amma-mother, appchhi-uncle, akka-sister, anna-brother, ajja-grandfather, ajji-grandmother, aayee-grandmother
He tells these words very clearly. Apart from these words he does not speak any other words. But when he is in mood babbles a lot. But if we tried to speak to him he does not show interest. my parents say that by now he should have started speaking. This is concerning to me. When he was born we have done hearing test. Test results were normal.

I wanted to get your opinion for these concerns.

From your description it looks like the physical and mental milestones are appropriate for age.
1. Almost all kiDs get temper tantrums. It is normal at this age.
2.Television mobiles gadgets should not be given for less than three year old. It prevents normal sensory learning. Avoid these. Use books and toys instead.
3. Restrict him from doing circling a lot. It might cause dizziness.
4. Speech may take 2 years too. But keep talking to him. Put him in play school at 2 years to help him interact with other kids.
