default
Posted on Fri, 24 Jun 2016
Question: Hi. I have been getting Headaches for a couple of weeks now. Felt fushed in the face yesterday but temp was normal. Light headed and dizzy all day yesterday and a little bit today. The nurse checked my pb yesterday and it was 163/98. Went to the chemist this morning and it was 147/92 and hr was 89 resting. Probably unrelated but my shoulder the other night went really hot for about 20mins as if someone had put heat rub on it. And my index finger was really cold all day yesterday. Something doesn't feel right. Should i be concerned. Thanks.

Brief Answer:
Consultation

Detailed Answer:
Hi XXXX, thanks for asking from HCM.

I can understand your concern. Your BP seems higher according to your age. It should remain below 140/90 mm Hg. So you should consult doctor for regular checkup of BP and rule out hypertension.
You can control BP to some extent by
: Decreasing salt intake (<2gm)
: Starting morning walk
: Avoiding mental and physical stress.
: Decrease intake of fat and junk food.

This high BP could be the reason behind your headache.

But you are also having complaints of abnormal heat-cold sensations, light headedness and dizziness. These symptoms could be due to

: Polyneuropathy
: Cervical spondylosis.

Peripheral Polyneuropathy - It can present as decreased sensation (touch, temperature, pain), tingling/numbness, abnormal sensations, mostly in extremities (feet, hands). It may cause autonomic disturbance like sudden flushing of face, peripheral swelling of limbs, irregular BP/pulse etc.

You can suspect it more if there is history of diabetes mellitus, past history of low vitamin B12/folic acid/calcium, working at some paint/pesticide factory, taking medication for long time (e.g. metronidazole).

Cervical spondylosis - It is cervical disc prolapse compressing spinal cord/nerves. It usually presents as neck pain radiating to shoulder, numbness/tingling/sudden hot or cold sensation in upper and lower limbs, muscular weakness, vertigo (light headedness/dizziness).

If you are having such symptoms, consult doctor.

Both conditions can be investigated by few investigations like

: MRI cervical spine
: Blood level of B12, folic acid,calcium, vitamin D3
: Blood level of sugar

After evaluation, it can be treated mostly by medication and rest. Specific treatment can be started by confirming diagnosis.

Hope it will help you. If still in doubt, do let me know. I would be happy to help.

Thanks. Take care.
