Question: Hi I have been having ongoing constant lightheadness for nearly three years now.... MRI all ok, Blood tests all ok, CT just found normal aging and some bone spurs, diet controlled diabetic and high blood pressure on coveram for nearly 7 years,,, have tried other BP pills but it makes dizzyness worse and my readings are really good on coverram eg today 124/76 and BSL 6.4 and was feeling really lightheaded so am sure this is not the cause...also neck pain where spine meats top of neck.... I am wondering if it could be pinched nerve or migraine and how to fix it am over it and sick of being sick its effecting me wanted to go out to any functions in case i get lightheaded

Brief Answer:
Pinched nerve in the neck could be the reason.

Detailed Answer:
Hi,

Thank you for posting your query.

I have noted your symptoms in detail. I also noted your blood sugar and BP readings and they are fine. Your current medication is also fine.

Normal MRI is a good news and it rules out any serious underlying brain disease.

I agree with you that a pinched nerve in the neck region could be responsible for your symptoms.

Initial treatment would consist of physical therapy, neck extension exercises, IFT and ultrasound therapy.

Medications such as betahistine tablets would also be useful.

Migraine is less likely as other symptoms such as headache, vomiting, sensitivity to light and sound are missing in your case.

I hope my reply has helped you.

I would be pleased to answer, if you have any follow up queries or if you require any further information.

Brief Answer:
You should describe your symptoms.

Detailed Answer:
Thank you for getting back and providing more information.

I agree with you that vestibular migraine can have similar features, however this condition is not so common.

When you see your neurologist in 4 weeks time, you should describe your symptoms in detail, as that is the most important step in making a correct diagnosis.

You should discuss the treatment options in detail too. For migraine, you would require preventive medications to reduce the severity and frequency of episodes. For pinched nerve, I have described the treatment above.

Best wishes,
Dr Sudhir Kumar MD DM (Neurology)
