What Causes Clotted Menstrual Bleeding While On Birth Control Pill?

Question: I have been on continuous birth control for endometriosis and ovarian cysts. I have not had a period in a year. About 2 months ago I started spotting every day. I do not need to use a tampon or pad, just a liner. At the beginning of what would be a normal cycle there is no spotting. Sometimes it is a little heavier and there is pain and blood clots or what looks like discharge with blood. I lost 25 lbs in 30 days and the bleeding started 25 days into that diet. I have history of my body not making enough progesterone. I have an appointment with obgyn, but I am starting to really worry about uterine cancer or gynocological cancers. What could be the cause of this? Have been on Microgestin (Junel) for four years with no issues. 34, no children, surgery to remove ovarian tumors 4 years ago and DNC for polyps

Abnormal uterine bleeding is a common side effect of all forms of hormonal contraception.

Although this bleeding is rarely dangerous, it is safer to evaluate the cause of bleeding.

Abnormal bleeding can also be a symptom of endometrial cancer, but OCPs are protective for endometrial cancer.

However, I would suggest you to go for tranvaginal ultrasound and endometrial biopsy to confirm.

Also, drugs like Advil can help to control bleeding.
A short term increase in estrogen dose can also help.

Progestin can increase the bleeding tendency.

Kindly discuss these issues with your doctor and do revert back.
